REQUEST FOR MORE INFORMATION
Dear Mr. Geewax:
On April 8, 2011, the Division of Water Quality (DWQ) received your application dated April 5, 2011 to
impact 176 square feet of Zone 1 and 799 square feet of Zone 2 to construct the proposed 4-bedroom residence
on the subject property. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below. The DWQ will require additional information in order to process
your application to impact protected isolated wetlands and/or streams on the subject property. Therefore, unless
we receive the additional information requested below, we will have to move toward denial of your application
as required by 15A NCAC 2H.0506 and 15A NCAC 2B. 0259(9). This project will be placed "on hold" as
incomplete until we receive this additional information. Please provide the following information so that we
may continue to review your project.
Additional Information Requested:
1. Impact Justification (Avoidance and Minimization)
This Office believes that the house labeled on the plans as proposed residence can be moved or
reconfigured to avoid the impacts to the protected Tar-Pamlico Riparian Buffers. Please revise the plans
to avoid the impacts.

2. Septic Approvals and Repair Fields
The plat does not indicate the location of repair field for the septic system nor give proof of septic approval
permits from Hyde County. Please indicate the location of the repair field on the plat and provide proof of
septic approvals.
3. Finished Floor Elevation of porches
The application did not provide the FFE for the proposed porch. Please indicate the FFE. Please note,
open wooded slatted decking that is at least 8 feet high with no impervious materials underneath would not
be considered a buffer impact per Clarification Memo 2007-002 (included)
4. Stormwater Management
Stormwater Management Controls must be shown on the plats as well as a proposed operation and
maintenance agreement for each BMP employed. Your narrative included the use of cisterns. A sample
Rainwater Harvesting O&M Agreement has been included for your use.
Mitigation
You are required to mitigate for impacts to the protected riparian buffers. You shall submit a
mitigation plan as required under 15A NCAC 02B .0259 to this office for approval. Mitigation plans
typically proposed include Buffer Restoration or Enhancement [15A NCAC 02B .0260(9)] or
Payment into the Riparian Buffer Restoration Fund [15A NCAC 02B .0260(7)].
Please respond within 30 days of the date of this letter by sending this information to me in writing at the DWQ
Washington Regional Office. If we do not hear from you within 30 days, we will assume that you no longer
want to pursue this project and we will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized (at this
