On August 3, 2010 the Division of Water Quality (Division) issued a final inspection letter for the restored areas of your
project referenced above. At that time the Division determined that the work within the failed relocated stream channels
had been completed as approved and considered the previously issued violation closed at that time. The restoration
approval and completion required that "The permittee shall visually monitor the restoration of Stream 1 monthly for 4
months and then quarterly for a total of 1 year of monitoring after construction completion." In addition, the approval
required that you report any problems with the site to the Division's attention immediately.
On May 6, 2011, Sue Homewood from this Division conducted a routine inspection of the restored stream and stormwater
management system (as required by DWQ Certification 07-2160). The stormwater management system was determined
to be in compliance with DWQ Certification #07-2160. However, during this inspection it was noted that Stream 1 has
eroded and de-stabilized to a point that requires significant repairs. David Shoaf, Airport Commission Maintenance
Supervisor, and Chris Weezorak, AVCON Engineering, explained that a stream restoration consultant had already been
contracted to repair the damage to the stream and that work on the repair was to begin the following week. Please note
that the failure to contact the Division upon discovery of the stream failure is a violation of the restoration approval terms.
The Division encourages the Airport Authority to re-evaluate the causes of the repeated stream failures and consider re-
design of the stream relocation. Should you choose to repair the stream as designed, please notify Sue Homewood upon
completion of the repairs. If you choose to redesign the stream, you must provide plans to the Division for review and
approval prior to conducting the redesign/repair. In additiona, the Division requires that monitoring be restarted upon
completion of the repairs. Therefore the permittee shall visually monitor the restoration of Stream 1 monthlv for 4
months and then quarterly for a total of 1 year of monitoring from the completion of the May 2011 repair. The
Division shall be notified immediately upon discovery of any failure of the stream stability during the monitoring
period. Upon completion of the monitoring period the Division shall be contacted for a final closeout inspection.
