This memo is a follow-up to our site visit of the 311 Bypass project in Guilford and Randolph
Counties on May 2, 2011. 1 appreciate the time you, Brian Smith, Bill Snell and NCDOT and
contractor staff spent with Andy Williams (USACE) and me discussing 401 sites for this project.
The following items are provided to help document ourjoint site visit efforts and to assist the
Division NCDOT staff with current project status documentation. There are sites that were not
visited on this date. Only sites visited are discussed below. They are discussed in the order
they were reviewed.
An inspection of Construction Stormwater records was performed while on site. More detail in
the documentation of SDOs was requested. Overall the project was in compliance with the
Construction Stormwater permit.
Permit Site 7:
No in channel work has been conducted.
Permit Site 5:
The culvert installation is nearly complete. The culvert bed has been filled with organic material
per the plans. We discussed the tie in on the upstream side. There is @ a 2 foot drop between
the undisturbed wetland and the low flow sill of the culvert. A channel is planned to connect the
inlet with the undisturbed portion of the wetland. The culvert inlet has been over excavated and
will need to be brought up to the level of the low flow sill. We determined that you could use a
mix of rock and organic soil to bring up the elevation. DWQ is concerned that a headcut will
develop when tying the channel into the existing wetland. This area needs to be monitored very
closely. If any instability occurs, please consult DWQ to determine the appropriate measures to
be taken.
The culvert outlet has also been overly excavated and will need to be brought up to the level of
the low flow sill. We determined that you could use a mix of rock and organic soil to bring up
the elevation.

Permit Site 4:
The wetland has been filled in, and we didn't visit the site.
Permit Site I:
Culvert extension has been completed. This site is in compliance with the 401 permit.
Permit Site 2:
Culvert extension has been completed. This site is in compliance with the 401 permit.
Permit Site 3:
This site is adjacent to a large pond. The fill slope was being brought up and lined with rock and
filter fabric. An additional pipe was added to the plans that was not shown on the permit
drawings. This site was in compliance with the 401 permit.
There are other sites in the project that I didn't see while on site. I will be back out in the next
couple of months to continue to monitor on-going changes to the permitted areas. I will also
want to look at the areas where you have addressed issues discussed in the memo. Again,
