remedation system to Air SpargeNapor Extraction (AS/SVE). Pilot test wells have been installed under
VV10300341 to investigate the potential for AS/SVE as a viable option for the continued remediation of
impacted groundwater at the subject site. If the pilot test is successful, Colonial would apply for a

On October 19 2017, staff from the MRO performed a compliance evaluation inspection of the
Colonial Pipeline GW remedition sytem under permit # NG510207. The system was originally
permited in July of 1904. The system equipment conitnues to perform to permit requirements.
The system is well maintained. The records that were reviewed during the site visit were in order and
up to date.
Under an N.O.I. dated from 1 /28/2016, Colonial Pipeline is investigating the pos ibilty of changing the
remedation system to Air Sparge/Vapor Extraction (AS/SV). Pilot test wells have been installed under
W10300341 to investigate the :potential for ASISVE as a viable option for the continued rernediation of
impacted groundwater at the subject site. If the pilot test is Successful, Colonial would apply for a
permit rescission of the current remediation system and a closure inspection would be performed to
insure the system is properly decommissioned.

Dear Mr. Tuviam
On September 21, 2015 afield technician for prism Labs collected a quarterly effluent sample as
required for the above reference permit. As you are aware the total lead result from that event was 36
ug/l, exceeding the permitlimit of 25 ug/l. Upon learning of the exeedance Colonial been an
investigation to determine the reason for this occurrence. The results of that investigation are as
follows;
Colonial requested that Prism Lab re -run the sample that had exceeded the limit. On the second run
the result was 37 ug/1.
Colonial collected a new effluent sample for total lead` on November 3, 2015. The result for that sample
was non -detect.

GENERAL:
The facility was last inspected on July 0, 2007, by Amber Lindon of the Mecklenburg County Water
Quality Program (MCWQP).
PERMIT:
The current permit became effective on October 12, 2011, and expires on September 30, 2016. The
permit authorizes the discharge of treated groundwater. The portion of the facility associated with the
NPDES treatment works consists of five (5) active groundwater recovery wells, an oil/water separator,
an equalization tank, a law profile tray -type air stripper, an iron filtration unit; a holding tank, a
particulate filter, and two (2) 1,00-pound granular activated carbon vessels connected in series. The
current permit properly describes the facility and appears adequate for the waste stream being treated.
RECORDSIREPORTS. ,
The general'; groundwater remediation permit does not require the permittee to complete and submit
Discharge Monitoring Reports DMR ); however, the permittee is required to maintain on -site records of
all discharge monitoring data. The on -site data was reviewed for the period of June 2010 through May
2011. Records, reports, and logs were clear and well maintained.
FACILITY SITE REVIEW:
All of the major components of the facility were in service and appeared to be operating properly at the
time of the inspection.
FLOW MEASUREMENT:
Discharges from the groundwater remediation system are intermittent. Effluent flew is measured using
an in -line totalizing; meter capable of producing instantaneous and total flow measurements. The meter
was last calibrated in February 2007. In order to ensure proper flow measurements, a flow meter
should be calibrated annually. The in -line totalizing meter must be calibrated immediately.
LABORATORY:
Chemical analyses of effluent samples is performed by Prism Laboratories, Inc. (tom Laboratory
Certification #40). Prism Laboratories, Inc. was not evaluated during this inspection.
EFFLUENT/RECEIVING WATERS:
The receiving waters were clear at the time of the inspection. No petroleum sheens were noted. The
facility discharges to an unnamed tributary to Gum Branch, which is a Class WS-Ilt water in the
Catawba River Basin.
SELF -MONITORING PROGRAM:
Discharge Monitoring Reports (iMRs) were reviewed for the period June 2010 through May 2011. The
reports indicate that the permittee met all permit effluent limits and monitoring frequencies during the
review period.
OPERATIONS & MAINTENANCE:
The facility and site appeared well operated and maintained. The facility and outflls are visually
inspected by Colonial Pipeline personnel to ensure proper operation and safety.

Per our telephone conversation on April 10, 2013 Colonial Pipeline Company (Colonial) initiated an
investigation into the above referenced eceedence. As you are aware, the effluent sample collected
from the system on March 14, 2013 contained 110 micrograms per liter (ug/L) of total lead, exceeding
the discharge limit of 25 ug/L. The effluent was re -sampled on April 5„ 2013 and the total lead result
was 2' ug/l.

concentrations of lead observed in the samples collected from each of the extraction wells were well
below the effluent limit of 2 /L.
The turbidity of the water in the effluent was believed to be the result of sludge / sediment build up in
the two GAC vessels. It appears that the initial lead eceedence may have been due to turbidity of the
samples. The sample results are summarized in the table below:

Mr. Christopher Elmore of the Mecklenburg County Water Quality Program (MCWQP)
last inspected the facility on May 12, 2006
PERMIT:
The current permit became effective on October 1, 2006, and expires on September 30,
2011. The permit authorizes the discharge of treated groundwater to an unnamed tributary of
Gum Branch, a class -IV water in the Catawba River Basin.
The portion of the facility associated with the NPDES treatment works consists of five
active groundwater recovery wells, an oil/water separator, an equalization tank, a low profile
tray -type air stripper, an iron filtration unit, a holding tank, a particulate filter, and two ,(ICE-
pound. granular activated carbon vessels connected in series.
The current permit properly describes the facility and appears adequate for the waste
stream being treated.
RECORDS/REPORTS:
The general groundwater remediation permit does not require the permittee to complete
d submit Discharge Monitoring Reports (DMRs); however, the permittee is required to
maintain ors -site records of all discharge monitoring data.. The on -site monitoring data was
reviewed for the period June 2006 through June 2007. The records were clear, organized, and
well maintained; no discrepancies were noted.
FACILITY SITE VIEW:
The facility discharges to an unnamed tributary to Gum Branch, a Class WS-IV water in the
Catawba River Basin. All of the major components of the facility were in service and appeared
to be operating properly at the time of the inspection.
FLOW MEASUREMENT
Discharges from the groundwater remediation system are intermittent. Effluent flow is
measured using an in -line totalizing meter capable of producing instantaneous and total flow
measurements. This is an acceptable instantaneous method for NPDES reporting'. The meter was
last calibrated in February 2007.
LABORATORY:
Prism Laboratories, Inc. (NC Laboratory Certification # 40 performs all effluent
analyses specified in the permit, Prism Laboratories was not evaluated during this inspection.

EFFLUENT/RECEIVING WATERS.
The facility was not discharging at the time of the inspection. I
noted in the receiving stream. No visual impacts were noted in the rec

producing instantaneous and total flow measurements.' This is an acceptable instantaneous
method for NPD ;S reporting. The meter was calibrated on July 14, 2005 by .Miller Meter
Services.
Discharges from the groundwater remediation system are intermittent.

groundwater. NCG51OOOO expires on September 30, 2006. Federal`(4O CFR 122.41) and North Carolina (1 A
NCAC 21-1.O1O5(e)) regulations require that permit renewal applications must be filed at least 180 days prior
to expiration of the current permit. If you have already mailed your renewal request, you may
disregard this notice.
To satisfy this requirement, the Division must receive your renewal request postmarked no later than
April 3. 2006. Failure to request renewal by this date may result in a civil assessment of at least $50.00.

Analysis of effluent samples is performed by Prism Laboratories, Inc. (NC Laboratory
Certification # 402). Prism Laboratories was not evaluated during this inspection.
EFTLUENURECEIVING WATERS:
The effluent was clear at the time of the inspection. No petroleum sheens or odors were
noted. No visual impacts were noted in the receiving stream.
SELF -MONITORING PROGRAM:
On -site monitoring data was reviewed for the period October 2001 through March 2003.
The data indicated that the facility was meeting permit effluent limits and monitoring frequencies
for all parameters during this period.
OPERATIONS & MAINTENANCE:
The facility and site appeared well operated and maintained. On -site personnel perform
routine preventativernaintenance on all components of the groundwater treatment system.

GENERAL:
e facility was last inspected on June 25, 2001 by Mr. Derrick Harris of the
Mecklenburg County Department of Enviromnental Protection (MCIEP). A Compliance
Evaluation Inspection (C ) Report was issued on July 17, 2001.
PERMIT:
The current permit became effective on October 1, 2001 and expires on September 30,
2006. The permit authorizes the discharge of treated groundwater to an unnamed tributary of
Gum Branch, a Class C stream. However, the unnamed tributary of Gum Branch is a waterbody
that should be listed as a Class WS-IV water.
The portion of the facility associated with the NPi ES treatment works consists of five'
active groundwater recovery wells, an oil/water separator, equalization tank, low profile; tray -type
air stripper, iron filtration unit, holding tank, particulate filter, `and two 1,800 pound granular
activated carbon vessels connected in series.
The current permit properly describes the facility and appears adequate for the waste
stream being treated.
RECORDS/REPORTS:
The facility is not rewired to complete and submit Discharge Monitoring Reports
(I ); however, the permit does require the facility to maintain on -site records of all discharge
monitoring data. The on -site monitoring data was reviewed for the period December 2000
through November 2001 e records were clear, organized, and wellmaintained; no
discrepancies were noted..
FACILITY SITEREVIEW:
The facility discharges to an unnamed tributary to Gum Branch, which then discharges to
the on -site retention pond. The retention pond discharges to an unnamed tributary to Gum
Branch, a Class WS-IV water in the Catawba River Basin. The effluent of the retention pond is
monitored under the NP ES Pen -nit NCO031038.
All of the major components of the facility were in service and appeared to be operating;
properly at the time of the inspection.
FLOW MEASUREMENT:

As directed by Colonial Pipeline Company (Colonial), Handex of the Carolinas,
Inc., ( land ) has prepared this response to the NCDENR Notice of Deficiency
(NOD) dated June 12, 2000, for the above -referenced site. The NOD followed e
Compliance Evaluation Inspection by the Mecklenburg County Department of
Environmental Protection conducted on June 7, 2000
The NOD resulted from an unsatisfactory rating for flow measurement duo to the
location of the flow meter. According to the Water Compliance Inspection
Report, the subject flaw meter is to be located on the effluent side of the
treatment system following all treatment. Currently, a flow meter is present prier

to the stripper. Colonial will address the noted deficiency by installing a flow
meter on the dawn -stream side of granular activated carbon polishing units
Every effort will be made to install the new flaw meter by June 30, 2000, Colonial
anticipates installation will take place no later than July 14, 2000.
