As you are aware the total lead result from that event was 36 ug/l, exceeding the permitlimit of 25 ug/l. Upon learning of the exeedance Colonial been an investigation to determine the reason for this occurrence. The results of that investigation are as follows; Colonial requested that Prism Lab re -run the sample that had exceeded the limit. On the second run the result was 37 ug/1. Colonial collected a new effluent sample for total lead` on November 3, 2015. The result for that sample was non -detect. RECORDSIREPORTS. , The general'; groundwater remediation permit does not require the permittee to complete and submit Discharge Monitoring Reports DMR ); however, the permittee is required to maintain on -site records of all discharge monitoring data. The on -site data was reviewed for the period of June 2010 through May 2011. Records, reports, and logs were clear and well maintained. FACILITY SITE REVIEW: All of the major components of the facility were in service and appeared to be operating properly at the time of the inspection. FLOW MEASUREMENT: Discharges from the groundwater remediation system are intermittent. Effluent flew is measured using an in -line totalizing; meter capable of producing instantaneous and total flow measurements. The meter was last calibrated in February 2007. In order to ensure proper flow measurements, a flow meter should be calibrated annually. The in -line totalizing meter must be calibrated immediately. LABORATORY: Chemical analyses of effluent samples is performed by Prism Laboratories, Inc. (tom Laboratory Certification #40). Prism Laboratories, Inc. was not evaluated during this inspection. EFFLUENT/RECEIVING WATERS: The receiving waters were clear at the time of the inspection. No petroleum sheens were noted. The facility discharges to an unnamed tributary to Gum Branch, which is a Class WS-Ilt water in the Catawba River Basin. SELF -MONITORING PROGRAM: Discharge Monitoring Reports (iMRs) were reviewed for the period June 2010 through May 2011. The reports indicate that the permittee met all permit effluent limits and monitoring frequencies during the review period. OPERATIONS & MAINTENANCE: The facility and site appeared well operated and maintained. A Compliance Evaluation Inspection (C ) Report was issued on July 17, 2001. PERMIT: The current permit became effective on October 1, 2001 and expires on September 30, 2006. The permit authorizes the discharge of treated groundwater to an unnamed tributary of Gum Branch, a Class C stream. However, the unnamed tributary of Gum Branch is a waterbody that should be listed as a Class WS-IV water. The portion of the facility associated with the NPi ES treatment works consists of five' active groundwater recovery wells, an oil/water separator, equalization tank, low profile; tray -type air stripper, iron filtration unit, holding tank, particulate filter, `and two 1,800 pound granular activated carbon vessels connected in series. The current permit properly describes the facility and appears adequate for the waste stream being treated. RECORDS/REPORTS: The facility is not rewired to complete and submit Discharge Monitoring Reports (I ); however, the permit does require the facility to maintain on -site records of all discharge monitoring data. The on -site monitoring data was reviewed for the period December 2000 through November 2001 e records were clear, organized, and wellmaintained; no discrepancies were noted.. FACILITY SITEREVIEW: The facility discharges to an unnamed tributary to Gum Branch, which then discharges to the on -site retention pond. The retention pond discharges to an unnamed tributary to Gum Branch, a Class WS-IV water in the Catawba River Basin. The effluent of the retention pond is monitored under the NP ES Pen -nit NCO031038. All of the major components of the facility were in service and appeared to be operating; properly at the time of the inspection. FLOW MEASUREMENT: Flow is measured on the effluent lute using an in -line totalizing flow meter capable of producing instantaneous total flow measurements. This is an acceptable instantaneous method for NPDES reporting. Discharges from the groundwater remediation system are intermittent. LABORATORY: Analysis of effluent samples is performed by Prism Laboratories, Inc. (NC Laboratory Certification # 402). Prism Laboratories was not evaluated during this inspection. EFFLUENT/RECEIVING WATERS: The effluent was clear ;at the time of the inspection. No petroleum hee or odors were noted. No visual impacts were noted in the receiving stream. SELF -MONITORING PROGRAM: On -site monitoring data was reviewed for the period December 2000 through November 2001. The data indicated that the facility was muting permit effluent limits and mo `toning frequencies for all parameters during this period. The NOD followed e Compliance Evaluation Inspection by the Mecklenburg County Department of Environmental Protection conducted on June 7, 2000 The NOD resulted from an unsatisfactory rating for flow measurement duo to the location of the flow meter. According to the Water Compliance Inspection Report, the subject flaw meter is to be located on the effluent side of the treatment system following all treatment.
