referenced project. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below.
Additional Information Requested:
® The signature on page 11 of 12 (Matt Swanson) is not valid as there is no mention of him
within the PCN as owner or agent. Please supply one (1) signed and dated original and four
(4) copies of actual applicant.
® DWQ requires five (5) complete application sets to begin the review and approval process.
Please supply four (4) more application sets.
® Check #1254 for $570 is being returned. Based on the reflected wetland impacts of 0.15 acres
the correct fee to remit is $240.
Please submit this information within 30 calendar days of the date of this letter. This letter only addresses
the application review and does not authorize any impacts to wetlands, waters or protected buffers.
