2j. Provide a brief description of this project (attach a site plan): The project will construct a +/-53,790 SF building with
approx.107 parking spaces, for use as a group care facility. The only stream impact will be for the driveway to access
the site. The site is bordered by streams on all accessible sides. The crossing is located on a tributary to the main
creek. No wetlands will be impacted. Neuse riparian buffer will be impacted in association with the stream crossing.
There will also be some disturbance to zone 2 of the NRB elsewhere along the driveway due to grading. These areas
will be revegetated.

From Raleigh take 1-40 East. Take exit 306 for US-70 toward Smithfield/Goldsboro. From 70E take the
Greenfield Parkway Exit. Turn right at the top of the ramp onto S. Greenfield Parkway. Turn left on Waterfield Drive.
Site is on the right before the creek crossing.
Please attach a map of the site location using USGS 1:24,000 topographic map and a copy of the latest bound
and published County soil survey with the project/site boundaries delineated. A site plan should also be included
for all projects other than stream and wetland determinations.
