Fichera 1551 Rock Quarry Rd Raleigh, NC 27610 Project Name: Rolesville High School Dear Mr. Fichera, Wake County On April 15, 2011, the Division of Water Quality DWQ received a request from Mr. Bob Zarzecki, of Soil & Environmental Consultants, PA, regarding a project known as "Rolesville High School" for acceptance into the Express Review Program. This letter advises you that your project will be accepted into the Express Review Program once the following items are received: 1) The application fee of $2,000.00 [401 Water Quality Certification with minor impacts + Riparian Buffer Authorization], made payable to the North Carolina Division of Water Quality]; 2) Five (5) complete and collated copies of the PCN Application and all documents we requested in the pre-application meeting (instead of providing five copies of the full size plans you may provide two copies of the full size plans along with three copies of the 11 X 17); 3) One (1) copy of the completed PCN Checklist (hiip://portal.ncdenr or web/wq/sw,p/ws/401/certsandpermits/apply/forms); 4) Two (2) copies of the stormwater management plan for the project and details for on-site diffuse flow provisions, if applicable (refer to b.9p://portal.ncdenr.or web/w /ws/su/bmp-manual); and 5) One (1) data CD of full size plans in TIFF Group 4 format (black and white, not grayscale or color). If the plans are too large to store in TIFF format, they can be stored in PDF. A pre-application meeting (to include applicant) is required prior to the submittal of your completed application package. The clock for this project will not start until receipt of the completed application package and required fee are received. If the impacts are greater than indicated on the Initial Interest Form and/or the services are different, you may be required to remit an additional fee.
