Sullins Dee Freeman Governor Director Secretary April 11, 2011 EXPRESS REVIEW ACCEPTANCE LETTER Wal-Mart Real Estate Business Trust Attn: Mr. Daniel Mallory, Vice President of Real Estate Design 2001 SE I Oh Street Bentonville, AR 72716 Subject Property: Wal-Mart Store No. 3036-00, Yanceyville STORMWATER MANAGEMENT PLAN (SMP) DWQ #: EXP 11-0189v2 Caswell County On April 9, 2011, the 401 Oversight/Express Permitting Unit of the DWQ received a request from Mr. Justin Houston of Kimley-Horn and Associates, Inc. for acceptance into the Express Review Program regarding a stormwater management plan (SMP) associated with the proposed development in Caswell County. This letter advises you that your project would be accepted into the Express Review Program when the following items are submitted: a. The application fee of $1000.00 made payable to the North Carolina Division of Water Quality [SMP with one (1) drainage area]; b. Two (2) complete and collated copies of the Express Review Stormwater Management Plan Application [http://portal.ncdenr.org/web/wq/swp/ws/401/eLcpress/fonns]; Two (2) collated copies of all supporting information pertaining to this project including i. Design calculations for the proposed BMP(s) sized for both on-site and any off-site drainage; ii. Most current BMP Supplement completed for each BMP including Design Summary and Required Items Check List along with all required items (available at http://portal.ncdenr.orp-/web/wq/ws/su/bmp-manual); iii. Completed, signed and notarized Operation & Maintenance Agreement(s) for the BMP(s), available at the same website in (ii) above. d. Two (2) collated copies of full size plans (with detailed construction information) including, but not limited to, the following: i. Existing conditions map of project area showing all stream and wetland features; ii. Proposed site grading and drainage plans showing existing and proposed contours, stream, wetland and buffer impacts (if any), storm drainage system - from inlets to outfalls, all impervious areas (buildings, roads, streets, amenities, parking, etc) and delineated drainage area to each proposed BMP; iii. Construction sequence and planting schedule (if applicable) for the proposed BMPs; e. One (1) data CD of full size plans in TIFF Group 4 format (black and white, not grayscale or color). If the plans are too large to store in TIFF format, they can be stored in PDF. Please note that a pre-application meeting is required prior to the submittal of your completed application. The pre-application meeting has been scheduled for Monday April 18, 2011 at 10:00am. The owner(s) of the project is/are required to attend the pre-application meeting, but they can waive this right. The clock for this project will not start until receipt of the completed application package and required fee. If the drainage areas exceed what is indicated on the Initial Interest Form and/or the services are different you may be required to remit an additional fee.
