Dear Mr. McMillan:
Please find enclosed a check for $240 for the application fee associated with the issuance
of a new 401 WQC for permanent stream impacts associated with the Crabtree Creek
North Bank 42" Emergency Sewer Replacement that were not included in the initial 401 -
WQC that was issued for this project (DWQ Project #07-1093, Ver. 2).
The additional information submitted to your office on March 11, 2011 documents the 67
If of permanent stream impacts, 68 if of temporary stream impacts and 0.294 acres of
temporary wetland impacts that resulted from the emergency replacement of the failing
42" sewer line.
Based on our conversation, the $240 application fee is the only thing needed for the new
401 WQC to be issued.
