This project is being submitted under the 401 Automated Payment Procedure. The State
Project Number is 6C.024083 and the permit fee is $240.00.

Culvert Replacement SR-1585 (Union Valley Rd) Columbus County
State Project Number: 6C.024083
The North Carolina Department of Transportation (NCDOT) has scheduled to clear, grade, drain,
base and pave SR-1585, Columbus County. Total project length is 0.15 miles. The roadway
footprint will be widened from 30 feet in maintenance limits to 60 feet, including 20 foot paved
roadway with 6 feet stabilized grass shoulders. This project will require the replacement of one
(1) wet cross line as shown below:

New Stream Impacts = 24 linear feet
Construction of this project will require impacts to the waters of the United States in the Lumber
River Basin. Permit site 41 is an unnamed tributary of Mill Branch (C;Sw). The project is
located within the 03040203 Hydrologic Unit, sub basin 03-07-58.
SR-1585. has been partially paved from SR-1002 (Old Lumberton Rd) for 0.4 miles. The
remaining 0.6 miles of this section of SR-1585 has been on hold because of Right of Way issues
with the sole property owner. The deterioration of the existing culvert necessitates replacement,
therefore a 0.15 mile section beginning at SR-1549 shall be paved and the culvert replaced. At
the present time, no compromise with the property owner can be reached for the remaining 0.45
mile.
Permit Site 91 is located at Station 54+68.91. The current culvert is 30 LF 18" RCP, NCDOT
proposes to increase the size to a 54 LF 24" RCP. This UT to Mill Branch is a perennial stream

keyed out utilizing the NC DWQ Stream form at a rating of 30.5. Upstream roadside ditches
merge together as the property owner has shifted the flow of water through the farm field. These
re-routed stream portions join with ground water to re-form the stream at the mouth of the
existing culvert, flow under SR-1585 and then continue on in a well developed stream channel
downstream. The downstream portion has very defined bed and banks with a stream width of
roughly 3 feet. There are also wetlands associated with this Headwater stream, but are located
outside of the area of disturbance. NCDOT will install silt fence to protect the wetland area and
place flagging on the post to caution workers.
RESOURCESTATUS
In general, there will be minor impacts proposed as a result of the replacement of the culvert
along SR=1585 (Union Valley Rd.) in Columbus County. This culvert site has undergone human
manipulation. A Nationwide Permit #14 / NCDENR 401 WQC 3820 is required for the project.
Endangered Species: Plants and animals with Federal classification of Endangered (E) or
Threatened (T) are protected under provisions of Section 7 and Section 9 of the Endangered
Species Act of 1973, as amended. As of September 22, 2010, the U.S. Fish and Wildlife Service
lists eight federally protected species for Columbus County.

A biological conclusion of "No Effect" was reached for all species during the February 15, 2011
survey for protected species. Protected species were not identified, nor, was the general habitat
for each species identified.
REGULATORY APPROVALS
Application is hereby made for a Department of the Army Nationwide Permit 14 / NCDENR 401
WQC 3820 as required by the above-described activities.
The National Register was reviewed in addition to conducting a field view of the project for
significant historic features. No structures were listed in the vicinity of the project.
The project impacts are minor in nature and should not remove or degrade existing water quality
uses. Pipe sizes proposed are the minimum necessary to create a safe highway while
accommodating the 25-year design storm. There is no record within the NCDOT District 3
Office (Columbus County) that the road experiences flood overtopping. By utilizing the erosion
control measures, downstream water quality standards will be protected. All appropriate BMP's
will be utilized during construction.
Attached is a Preconstruction Notification Form, plan sheets, marked county road map, quad
location map and photographs.

Road width is currently 18 feet within a 30ft maintenance limit. Forested land and farm fields, with portions having been
recently logged.

By Statute, NCDOT is required to pave all unpaved secondary roads. To ensure that the roadway meets current safety
standards, the road will be paved to a width of 20 feet, 6 foot grass shoulders will be installed on each side and 3:1 side
slopes will be provided within the 60 foot right of way.
3e. Describe the overall project in detail, including the type of equipment to be used:
Bring SR 1585 up to current safety standards, ie. 20 foot paving with 6 foot stabilized shoulders and 3:1 side slopes within
a 60 foot right of way. The sole property owner has refused to give Right of Way for the remaining 0.45 mile that is
unpaved. He has stated that until his death NCDOT will not have access to finish paving. The exisiting culvert is
deteriorated and needs to be replaced. Therefore from SR 1549, to a point 282ft beyond the culvert shall be brought up
to current safety standards. Impacts will be minimized as much as possible. The culvert will be replaced utilizing a track-
hoe excavator. While front end loaders will place fill material. Eventually a motor grader will finish grade with project prior
to paving.
