Description for property for stabilization wall for 7898 Bankhead Rd.

The property if vacant there was an old trailer on the property when we purchased it but we
have removed the trailer. We are requesting permission to build a Everlock retaining wall to
stabilize the shoreline which is in really bad shape. There are no trees or other vegetation near
the shoreline to be removed or distrubed. The lot is grassed over and the grass will be replaced
when the wall is finished. I have enclosed the materials used and descriptions from the
contractor who will be building the wall. If you have any questions please contact me at 276-

Certification to impact FERC (Federal Energy Regulatory Commission) regulated lake shore or
" bottom under the U.S. Army Corps of Engineers Permit GP30. This Application cannot be used

`This bid is for Standard Gray Block. There will be an additional charge for any change in block color
or style.
Our bid shall include all necessary materials (block, caps, grid, gravel, rip rap, stairs, and labor,
excavation) to begin and finish said retaining wall(s).
S & S Retaining Walls, Inc. will guarantee the wall(s) against defects in materials and workmanship.
This excludes heavy machinery (mowers, tractors, backhoes, etc.) used behind the walls and natural
acts (fallen trees, flash floods, tornadoes, etc.).
• The wall quantities assume the use of onsite soil in the reinforced, retained and foundation
zones of the wall with an assumed friction angle of 28 degrees or greater and moist unit weight
of 120 pcf.
• The long-term ground water is assumed to be well below the bottom of the wall.
• All quantities given represent in-place compacted quantities and do not assume anv waste,
overlap, shrinkage, swelling or over-excavation.
• Global stability was not considered in the preliminary design and should be calculated for the
final design.
• This information provided is for estimating purposes only.
• All designs should be reviewed and approved by a registered professional engineer in which the
quantities may vary and therefore a change in price may occur.
