The following are enclosed: • Application • Annual Budget for Collection System • Comprehensive Collection System Map • Response Action Plan • Pump Station List & Info (see map) • Contingency Plan • High Priority Lines List (see map) Please note the following: • There is only one pump station (duplex) and the collection system is small and relatively new. Additionally, there are no future plans to expand the system and there is no collection system component expected to need replacement within the next ten years. Therefore, the Town does not have a sewer system capital improvement plan. However, the Town has a $75,000 capital reserve account to address major system repairs. • Gamewell's collection system is 100%tributary to the City of Lenoir' collection system and therefore the City of Lenoir's Sewer User Ordinance (SUO) applies. Failure to submit all required items will necessitate additional processing and review time. For more information, visit the System-wide Collection System Permitting website: General—When submitting an application to the Pretreatment,Emergency Response,&Collection Systems(PERCS)Unit, please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of requested additional information. The Applicant shall submit one original and one copy of the application and supporting documentation. A. Cover Letter O Submit a cover letter listing all items and attachments included in the permit application package B. No Application Fee Required ➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit > The appropriate annual fee for systemwide wastewater collection system permits may be found at: > Annual Non-Discharge Fees C. System-Wide Wastewater Collection System (FORM: CSA 04-16)Application: O Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04- 16) application. Any unauthorized content changes to this form shall result in the application package being returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long as the attachments are numbered to correspond to the section and item to which they refer. O If the Applicant Type in Section I.3 is a Privately-Owned Public Utility, provide the Certificate of Public Convenience and Necessity(CPCN)from the North Carolina Utilities Commission demonstrating the Applicant is authorized to hold the utility franchise for the area to be served by the wastewater collection system,or ❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an application for a franchise has been received and that the service area is contiguous to an existing franchised area or that franchise approval is expected. Duke Power Outage (800) 769-3766 Account# 1837455338 RESPONSE ACTION PLAN SPILL RESPONSE PLAN SSO - Sanitary Sewer Overflow ORC - Operator in Responsible Charge 1. Upon discovery of a SSO and/or spill, immediately notify the ORC or their backup and the town administrator. 2. Use the appropriate equipment to correct the incident as soon as possible. 3. Determine the amount of the SSO and/or spill. Check to see if sewage has reached surface waters. 4. Document the SSO and/or spill using the standard Division of Water Resources SSO Reporting form (Form CS-SSO). 5. If it is determined the spill was in excess of 1,000 gallons to the ground or the spill contacted surface waters, the ORC or back up will report the SSO and/or spill to the Division of Water Resources Regional Office as soon as possible, but not longer than 24 hours @ 828-296-4500 or after hours @ 800-858-0368. SSO RESPONSE TIME REQUIREMENTS Any employee who is the primary ORC or backup (on-call) ORC must be no more than a 30-minute drive from the Town of Gamewell. In the event of a personal conflict, the primary ORC must inform the backup ORC so that other arrangements can be made. In the event of a SSO and/or spill during a weekend or holiday, the primary or on-call ORC must be notified immediately. If the ORC cannot be reached, the backup ORC must be contacted as soon as possible as to insure the SSO and/or spill can be stopped and reported as described above. POST SPILL CLEANUP Post spill cleanup once a sewer spill has occurred involves the following steps: • Area should be cleaned of any debris and the material must be disposed of properly. • Lime must be spread over the entire area. • Apply grass seed and straw to the affected rea. CONTINGENCY PLAN FOR PUMP FAILURE: Contact Smart Electric (828) 874-2875 to respond. If the problem is electrical, they can repair. If the problem is mechanical, contact Clearwater at 828-855-3182 or Carolina Lift Stations at 803-628-0742. If both pumps fail, contact one of the listed septic tank companies to pump and haul until repairs can be made. CONTINGENCY PLAN FOR LINE BLOCKAGE: Manhole Blockage : Contact septic tank hauler to pump MH down and remove blockage. Line Blockage : Contact Roto Rooter to rod line.
