1. THE CONTRACTOR SHALL VISIT THE SITE TO INFORM AND SATISFY HIMSELF OF ALL
EXISTING CONDITIONS PRIOR TO SUBMITTING HIS BID. ANY DISCREPANCY BETWEEN
THE SITE AND THOSE CONDITIONS REPRESENTED ON THE DRAWINGS SHALL BE
BROUGHT TO THE ATTENTION OF THE OWNER AND THE ENGINEER IMMEDIATELY.
2. ALL EXISTING UTILITIES AND/OR STRUCTURES INDICATED ON THE DRAWINGS ARE BASED
ON A FIELD SURVEY. THE EXISTING IMPROVEMENTS SHOWN ON THESE DRAWINGS MAY
NOT BE ALL-INCLUSIVE. THE CONTRACTOR SHALL TAKE PRECAUTIONARY MEASURES TO
PROTECT THE UTILITY LINES AND OTHER IMPROVEMENTS SHOWN HEREON AND ANY
OTHER EXISTING LINES NOT OF RECORD OR NOT SHOWN ON THESE PLANS, UNLESS
THEY ARE SHOWN TO BE REMOVED OR RELOCATED. THE CONTRACTOR SHALL BE
RESPONSIBLE FOR MAKING ARRANGEMENTS FOR THE PROTECTION, RELOCATION OR
REMOVAL OF CONFLICTING EXISTING UTILITIES AND SHALL BE FULLY RESPONSIBLE FOR
ANY AND ALL DAMAGES RESULTING FROM HIS FAILURE TO LOCATE AND PRESERVE ANY
AND ALL UNDERGROUND UTILITIES. BEFORE BEGINNING WORK THE CONTRACTOR
SHALL CONTACT THE N.C. ONE -CALL CENTER AT 811 TO HAVE ALL EXISTING UTILITIES
LOCATED.
3. THE LOCATION AND ELEVATIONS OF IMPROVEMENTS, TO BE MET OR CROSSED BY
WORK, SHALL BE CONFIRMED BY FIELD MEASUREMENT, SUFFICIENTLY AHEAD OF
CONSTRUCTION TO PERMIT REVISIONS TO THE PLANS, IF REVISIONS ARE NECESSARY
DUE TO ACTUAL LOCATION OF EXISTING UTILITIES.
4. THE CONTRACTOR SHALL ASSUME SOLE AND COMPLETE RESPONSIBILITY FOR JOB SITE
CONDITIONS DURING THE COURSE OF CONSTRUCTION OF THIS PROJECT, INCLUDING
SAFETY OF ALL PERSONS AND PROPERTY. THIS REQUIREMENT SHALL APPLY
CONTINUOUSLY AND NOT BE LIMITED TO NORMAL WORKING HOURS. THE CONTRACTOR
SHALL DEFEND, INDEMNIFY, AND HOLD THE OWNER AND THE ENGINEER HARMLESS
FROM ANY AND ALL LIABILITY, REAL OR ALLEGED IN CONNECTION WITH PERFORMANCE
OF WORK ON THIS PROJECT, EXCEPTING FOR LIABILITY ARISING FROM THE SOLE
NEGLIGENCE OF THE OWNER OR THE ENGINEER.
5. THE CONTRACTOR SHALL VERIFY THAT ALL REQUIRED PERMITS AND APPROVALS HAVE
BEEN OBTAINED FROM THE APPROPRIATE AUTHORITIES BEFORE BEGINNING WORK ON
THE PROJECT. APPROVED PLANS, APPROVAL LETTERS OR OTHER CERTIFICATES OF
APPROVAL SHALL BE MAINTAINED ON THE SITE AT ALL TIMES DURING CONSTRUCTION.
6. CLEARING AND GRUBBING DEBRIS SHALL BE DISPOSED OF OFFSITE IN A PERMITTED
LANDFILL FACILITY. DISPOSAL ON THE PIPELINE EASEMENT OR IN OTHER LOCATIONS IS
NOT ALLOWED.
7. GAS MAIN INSTALLATION SHALL BE BY OPEN CUT UNLESS SHOWN OTHERWISE ON THE
DRAWINGS. THE ENTIRE WIDTH OF THE RIGHTS -OF -WAY, EASEMENTS AND WORK
SPACES SHALL BE CLEARED AND GRUBBED UNLESS SHOWN OTHERWISE ON THE
DRAWINGS. ALL CLEARING DEBRIS - STUMPS, TREES, TOPS, BRUSH, ETC. SHALL BE
REMOVED FROM THE SITE AND DISPOSED OF IN A LEGAL MANNER.
8. AREAS SHOWN AS ' TREE CUTTING ONLY - NO GRUBBING' SHALL BE CUT AT NO MORE
THAN 12 INCHES ABOVE THE GROUND. STUMPS SHALL BE LEFT IN PLACE. LOGGING
MATS OR OTHER SIMILAR MEASURES SHALL BE USED AS NEEDED TO MINIMIZE GROUND
DISTURBANCE. ALL CLEARING DEBRIS - TREE TOPS, BRUSH, ETC. SHALL BE REMOVED
FROM THE SITE AND DISPOSED OF IN A LEGAL MANNER.
9. SEE PLANS BY NORTHSTAR ENERGY SERVICES FOR PIPING LAYOUT AND INSTALLATION
REQUIREMENTS ALSO FENCING AND GATE DETAILS.
10.SURFACE MATERIAL FOR STATION AREA SHALL BE 6" THICK #57 WASHED STONE. STONE
SHALL BE PLACED ON NON -WOVEN GEOTEXTILE FABRIC.
11.IF FENCING REMOVAL AND REPLACEMENT IS REQUIRED FOR INSTALLATION OF THE GAS
MAIN, THE CONTRACTOR SHALL PROTECT REMOVED FENCING MATERIAL DURING
CONSTRUCTION. ANY FENCING MATERIAL THAT IS DAMAGED SHALL BE REPLACED AT
THE CONTRACTORS EXPENSE WITH NEW MATERIAL OF LIKE SIZE, GAUGE, FINISH, ETC.
AS THE EXISTING MATERIAL. EXISTING FENCE POSTS THAT ARE SET IN CONCRETE SHALL
BE REPLACED WITH NEW POSTS AS DETAILED. DISPOSAL OF DAMAGED MATERIALS IS
THE RESPONSIBILITY OF THE CONTRACTOR.

EROSION CONTROL NOTES
1. THE CONTRACTOR SHALL CONFIRM THAT ALL REQUIRED PERMITS AND APPROVALS
HAVE BEEN OBTAINED FROM THE APPROPRIATE AUTHORITIES PRIOR TO BEGINNING
ANY WORK ON THE PROJECT. THE APPROVED PLANS AND THE CERTIFICATE OF PLAN
APPROVAL SHALL BE MAINTAINED ON THE SITE AT ALL TIMES.
2. PIEDMONT NATURAL GAS WILL HAVE A FULL-TIME EROSION CONTROL INSPECTOR
ONSITE. THE RESPONSIBILITY OF THE PNG INSPECTOR IS TO ENSURE COMPLIANCE
WITH THE APPROVED PLANS. THE PNG INSPECTOR DOES NOT HAVE THE AUTHORITY
TO CHANGE THE REQUIREMENTS OR SCOPE OF THE EROSION CONTROL WORK ON THE
PROJECT. IF PERMIT IS REQUIRED APPROVAL AUTHORITY FOR EROSION CONTROL
PLAN CHANGES LIES SOLELY WITH NCDEQ, IF NO PERMIT IS REQUIRED APPROVAL
AUTHORITY FOR EROSION CONTROL PLAN CHANGES LIES SOLELY WITH THE
ENGINEER.
3. AN ONSITE PRE -CONSTRUCTION MEETING WITH THE CONTRACTOR, THE PNG EROSION
CONTROL INSPECTOR, NCDEQ EROSION CONTROL INSPECTOR IF PERMIT IS REQUIRED
AND THE OWNER'S REPRESENTATIVE WILL BE REQUIRED PRIOR TO BEGINNING WORK.
4. THE NORTH CAROLINA SEDIMENTATION POLLUTION CONTROL PROGRAM IS
PERFORMANCE ORIENTED, REQUIRING PROTECTION OF NATURAL RESOURCES AND
ADJOINING PROPERTIES. THE EROSION AND SEDIMENT CONTROL MEASURES AND
DEVICES SHOWN ON THE PLANS ARE INTENDED AS A GUIDE. IF THESE MEASURES DO
NOT CONTROL EROSION AND SEDIMENT AS INTENDED, ADDITIONAL MEASURES SHALL
BE ADDED AS NEEDED, OR AS INSTRUCTED BY THE EROSION CONTROL INSPECTOR.
5. THE CONTRACTOR IS RESPONSIBLE FOR FOLLOWING THE SEQUENCE OF
CONSTRUCTION.
6. INSTALL AND MAINTAIN ALL EROSION AND SEDIMENT CONTROL MEASURES AND
DEVICES AS SHOWN ON THE PLANS.
7. THE LIMITS OF DISTURBANCE AND CLEARING LIMITS, IF APPLICABLE, SHALL FOLLOW
THE EASEMENT LINES UNLESS NOTED OTHERWISE ON THE PLANS. SILT FENCE SHALL
BE PLACED ON THE LIMITS OF DISTURBANCE LINE, BUT MAY BE SHOWN OFFLINE ON
THE PLANS FOR CLARITY.
8. SILT FENCE OUTLET DEVICES ARE SHOWN AT THE LOW SPOTS BASED ON THE
ELEVATION CONTOURS. SHIFT THESE OUTLET DEVICES AS NEEDED IN THE FIELD TO
LOCATE THEM IN THE ACTUAL LOW SPOTS.
9. ALL DISTURBED AREAS SHALL BE SEEDED AND MULCHED IN ACCORDANCE WITH THE
SEEDING SCHEDULE WITH LIMITATIONS AS FOLLOWS. GIVEN THE NATURE OF THE
PROJECT, TEMPORARY AND PERMANENT SEEDING MIXTURES HAVE BEEN COMBINED
AND SHALL BE APPLIED IN A SINGLE OPERATION.
10. ALL EROSION AND SEDIMENT CONTROL DEVICES SHALL REMAIN IN PLACE UNTIL ALL
DISTURBED AREAS ARE REVEGETATED AND THE EROSION CONTROL INSPECTOR
APPROVES THE REMOVAL OF THE DEVICES. ALL TEMPORARY EROSION AND SEDIMENT
CONTROL DEVICES SHALL BE REMOVED UPON APPROVAL BY THE EROSION CONTROL
INSPECTOR.
11. THE CONTRACTOR IS RESPONSIBLE FOR INSTALLING ALL MEASURES AND DEVICES
SHOWN ON THE PLANS, AND FOR COMPLYING WITH THE PROVISIONS AND CONDITIONS
OF THE PROJECT SPECIFIC EROSION CONTROL PERMIT.
GRADING NOTES
1. ALL GRADING ACTIVITIES SHALL BE CONTAINED WITHIN THE LIMITS OF DISTURBANCE
SHOWN ON THE PLANS.
2. ALL AREAS TO BE GRADED SHALL BE STRIPPED TO REMOVE ALL TOPSOIL, STUMPS,
ROOTS, ORGANICS AND OTHER UNSUITABLE MATERIAL. AFTER STRIPPING, EXPOSED
SUBGRADE IN BUILDING AND PAVEMENT AREAS TO BE FILLED SHALL BE PROOF
ROLLED IN THE PRESENCE OF THE GEOTECHNICAL ENGINEER. IF UNSTABLE AREAS
ARE ENCOUNTERED THAT ARE THE RESULT OF WET SOIL CONDITIONS, REASONABLE
EFFORT SHALL BE MADE TO MECHANICALLY DRY THOSE SOILS. IF UNSUITABLE SOILS
ARE ENCOUNTERED THEY SHALL BE UNDERCUT TO FIRM MATERIALS AS DIRECTED BY
THE GEOTECHNICAL ENGINEER, AND BACKFILLED WITH CONTROLLED FILL.
REASONABLE DRYING EFFORT AND UNSUITABLE SOILS ARE DEFINED IN THE
EARTHWORK SPECIFICATIONS.
3. CONTROLLED FILL SHALL BE BENCHED INTO THE SIDES OF SLOPES STEEPER THAN 4:1
(H:V) TO IMPROVE THE BOND BETWEEN THE FILL AND THE UNDISTURBED SOIL. EACH
BENCH CUT INTO A SLOPE SHOULD BE AT LEAST 12 INCHES BUT NOT MORE THAN 60
INCHES HIGH. THE BENCHES SHALL BE WIDE ENOUGH TO ALLOW OPERATION OF THE
COMPACTION EQUIPMENT WITHIN THE BENCHED AREA SO THAT FILL CAN BE PLACED
ON THE BENCH AND OVER THE REST OF THE FILL IN CONTINUOUS LAYERS.
4. FILL SLOPES SHALL BE CONSTRUCTED BY PLACING CONTROLLED FILL IN HORIZONTAL
LIFTS WHICH EXTEND SLIGHTLY BEYOND THE SLOPE FACE. EACH LIFT OF FILL SHALL
BE COMPACTED TO THE SPECIFIED MINIMUM DENSITY BEFORE PLACING THE NEXT LIFT.
THE COMPACTION EQUIPMENT SHOULD TRAVEL PARALLEL TO THE SLOPE FACE TO
ENSURE PROPER COMPACTION. ONCE THE SLOPE IS COMPLETED IT SHALL BE
TRACKED WITH A DOZER TO MINIMIZE THE POTENTIAL FOR EROSION.
5. CONTROLLED FILL FOR STRUCTURAL AREAS SHALL BE COMPACTED TO 95% OF THE
STANDARD PROCTOR MAXIMUM DRY DENSITY (ASTM - D698), UNLESS OTHERWISE
NOTED ON THE PLANS, IN THE SPECIFICATIONS, OR GEOTECHNICAL REPORT. FILL
UNDER BUILDINGS AND PAVED AREA SHALL BE COMPACTED TO 98% STANDARD
PROCTOR IN THE TOP 12" OF SUBGRADE. COMPACTION TESTS SHALL BE TAKEN TO
SUBGRADE AND MINIMUM 15 FEET BEYOND LIMITS OF PAVED AREAS OR BUILDING
FOOTPRINTS AREAS. FREQUENCY OF COMPACTION TESTING SHALL BE DONE IN
ACCORDANCE WITH THE WRITTEN SPECIFICATIONS.
6. PERMANENT CUT AND FILL SLOPES SHOWN TO RECEIVE EROSION CONTROL MATTING
SHALL BE TRACKED WITH A DOZER PRIOR TO INSTALLING THE MATTING.
STORM DRAINAGE NOTES NOTES
1. ALL STORM DRAINAGE CONSTRUCTION SHALL CONFORM TO NCDOT STANDARDS AND
SPECIFICATIONS.
2. ALL STORM SEWER PIPE SHALL BE REINFORCED CONCRETE PIPE (RCP) (15" MIN.
DIAMETER) UNLESS OTHERWISE NOTED. RCP SHALL BE MINIMUM CLASS III WITH BUTYL
RUBBER MASTIC JOINTS UNLESS NOTED OTHERWISE ON THE PLANS.
3. HIGH DENSITY POLYETHYLENE (HDPE): PIPE AND FITTINGS SHALL COMPLY WITH THE
LATEST EDITIONS OF AASHTO M252 (FOR TUBING), AND M294 (FOR PIPE). PIPE AND
FITTINGS SHALL BE DOUBLE WALL WITH OUTER CORRUGATIONS AND SMOOTH
INTERIOR. PIPE AND FITTINGS SHALL HAVE WATER TIGHT RUBBER GASKET JOINTS.
PIPE UNDER PAVEMENTS SHALL BE BEDDED AND ENCASED WITH #57 OR #67 WASHED
STONE. TRENCH BACKFILL SHALL COMPLY WITH THE REQUIREMENTS FOR
CONTROLLED FILL. HAUNCHING AND BACKFILL MATERIAL FOR PIPE OUTSIDE TRAFFIC
AREAS MAY BE SOIL THAT IS EXCAVATED FROM THE PIPE TRENCH.
4. ALL RIP RAP FOR CHANNEL LINERS AND INLET AND OUTLET PROTECTION SHALL BE
INSTALLED WITH A WASHED STONE BEDDING LAYER AND GEOTEXTILE FABRIC
UNDERLINER AS DETAILED. RIP RAP SHALL BE INSTALLED SUCH THAT THE FINISHED
SURFACE OF THE LINER IS AT FINISHED GRADE.
5. THE CONTRACTOR SHALL REPAIR AND/OR REPLACE ANY EXISTING STORM DRAINAGE
LINES AND/OR STRUCTURES THAT ARE DAMAGED DURING THE CONSTRUCTION OF
THIS PROJECT. ALL REPAIR MEASURES ARE TO BE AT THE CONTRACTORS EXPENSE
AND SHALL MEET THE STANDARDS AND SPECIFICATIONS AS NOTED WITHIN THE STORM
DRAINAGE NOTES ON THIS SHEET. PIPE AND STRUCTURES THAT ARE REPLACED SHALL
BE REPLACED WITH SAME TYPE AS EXISTING.
MODIFICATIONS AND PERFORMANCE RESERVATIONS FROM NCDEQ
1. THE DEVELOPER IS RESPONSIBLE FOR THE CONTROL OF SEDIMENT ON -SITE. IF THE
APPROVED EROSION AND SEDIMENTATION CONTROL MEASURES PROVE INSUFFICIENT,
THE DEVELOPER MUST TAKE THOSE ADDITIONAL STEPS NECESSARY TO STOP
SEDIMENT FROM LEAVING THIS SITE (NCGS 113A-57(3)). EACH SEDIMENT STORAGE
DEVICE MUST BE INSPECTED AFTER EACH STORM EVENT (NCGS 113A-54.1(E)).
MAINTENANCE AND/OR CLEAN OUT IS NECESSARY ANYTIME THE DEVICE IS AT 50%
CAPACITY. ALL SEDIMENT STORAGE MEASURES WILL REMAIN ON SITE AND
FUNCTIONAL UNTIL ALL GRADING AND FINAL LANDSCAPING OF THE PROJECT IS
COMPLETE (15A NCAC 04B .0113).
2. ALL EXISTING DITCHES ON THIS PROJECT SITE ARE ASSUMED TO BE LEFT
UNDISTURBED BY THE PROPOSED DEVELOPMENT UNLESS OTHERWISE NOTED. THE
REMOVAL OF VEGETATION WITHIN ANY EXISTING DITCH OR CHANNEL IS PROHIBITED
UNLESS THE DITCH OR CHANNEL IS TO BE REGRADED WITH SIDE SLOPES OF 2
HORIZONTAL TO 1 VERTICAL OR LESS STEEP (15A NCAC 04B .0124(D)). BANK SLOPES
MAY BE MOWED, BUT STRIPPING OF VEGETATION IS CONSIDERED NEW EARTH WORK
AND IS SUBJECT TO THE SAME EROSION CONTROL REQUIREMENTS AS NEW DITCHES
(NCGS 113A-52(6)).
3. THE DEVELOPER IS RESPONSIBLE FOR OBTAINING ALL PERMITS AND APPROVALS
NECESSARY FOR THE DEVELOPMENT OF THIS PROJECT PRIOR TO THE
COMMENCEMENT OF THIS LAND DISTURBING ACTIVITY. THIS COULD INCLUDE OUR
AGENCY'S STORMWATER REGULATIONS AND THE DIVISION OF WATER RESOURCES'
ENFORCEMENT REQUIREMENTS WITHIN SECTION 401 OF THE CLEAN WATER ACT, THE
U.S. ARMY CORPS OF ENGINEERS' JURISDICTION OF SECTION 404 OF THE CLEAN
WATER ACT, THE DIVISION OF COASTAL MANAGEMENT'S CAMA REQUIREMENTS, THE
DIVISION OF SOLID WASTE MANAGEMENT'S LANDFILL REGULATIONS, THE
ENVIRONMENTAL PROTECTION AGENCY AND/OR THE U.S. ARMY CORPS OF ENGINEERS'
JURISDICTION OF THE CLEAN WATER ACT, LOCAL COUNTY OR MUNICIPALITIES'
ORDINANCES, OR OTHERS THAT MAY BE REQUIRED. THIS APPROVAL CANNOT
SUPERSEDE ANY OTHER PERMIT OR APPROVAL.
4. ADEQUATE AND APPROPRIATE MEASURES MUST BE PROPERLY INSTALLED
DOWNSTREAM, WITHIN THE LIMITS OF DISTURBANCE, OF ANY LAND DISTURBING
ACTIVITY TO PREVENT SEDIMENT FROM LEAVING THE LIMITS OF DISTURBANCE,
ENTERING EXISTING DRAINAGE SYSTEMS, IMPACTING AN ON -SITE NATURAL

EQUIPMENT AND VEHICLE MAINTENANCE
1. Maintain vehicles and equipment to prevent discharge of fluids.
2. Provide drip pans under any stored equipment.
3. Identify leaks and repair as soon as feasible, or remove leaking equipment from the
project.
4. Collect all spent fluids, store in separate containers and properly dispose as
hazardous waste (recycle when possible).
5. Remove leaking vehicles and construction equipment from service until the problem
has been corrected.
6. Bring used fuels, lubricants, coolants, hydraulic fluids and other petroleum products
to a recycling or disposal center that handles these materials.
LITTER, BUILDING MATERIAL AND LAND CLEARING WASTE
1. Never bury or burn waste. Place litter and debris in approved waste containers.
2. Provide a sufficient number and size of waste containers (e.g dumpster, trash
receptacle) on site to contain construction and domestic wastes.
3. Locate waste containers at least 50 feet away from storm drain inlets and surface
waters unless no other alternatives are reasonably available.
4. Locate waste containers on areas that do not receive substantial amounts of runoff
from upland areas and does not drain directly to a storm drain, stream or wetland.
5. Cover waste containers at the end of each workday and before storm events or
provide secondary containment. Repair or replace damaged waste containers.
6. Anchor all lightweight items in waste containers during times of high winds.
7. Empty waste containers as needed to prevent overflow. Clean up immediately if
containers overflow.
8. Dispose waste off -site at an approved disposal facility.
9. On business days, clean up and dispose of waste in designated waste containers.
PAINT AND OTHER LIQUID WASTE
1. Do not dump paint and other liquid waste into storm drains, streams or wetlands.

1. Do not discharge concrete or cement slurry from the site.
2. Dispose of, or recycle settled, hardened concrete residue in accordance with local
and state solid waste regulations and at an approved facility.
3. Manage washout from mortar mixers in accordance with the above item and in

4. Install temporary concrete washouts per local requirements, where applicable. If an
alternate method or product is to be used, contact your approval authority for

sections. Stormwater accumulated within the washout may not be pumped into or

can be shown that no other alternatives are reasonably available. At a minimum,
install protection of storm drain inlet(s) closest to the washout which could receive
spills or overflow.
7. Locate washouts in an easily accessible area, on level ground and install a stone
entrance pad in front of the washout. Additional controls may be required by the
approving authority.
8. Install at least one sign directing concrete trucks to the washout within the project
limits. Post signage on the washout itself to identify this location.
9. Remove leavings from the washout when at approximately 75% capacity to limit
overflow events. Replace the tarp, sand bags or other temporary structural
components when no longer functional. When utilizing alternative or proprietary
products, follow manufacturer's instructions.
10. At the completion of the concrete work, remove remaining leavings and dispose of
in an approved disposal facility. Fill pit, if applicable, and stabilize any disturbance
caused by removal of washout.
HERBICIDES, PESTICIDES AND RODENTICIDES
1. Store and apply herbicides, pesticides and rodenticides in accordance with label
restrictions.
2. Store herbicides, pesticides and rodenticides in their original containers with the
label, which lists directions for use, ingredients and first aid steps in case of
accidental poisoning.
3. Do not store herbicides, pesticides and rodenticides in areas where flooding is
possible or where they may spill or leak into wells, stormwater drains, ground water

1. FIBER FILTRATION TUBES (FFT) SHALL BE ENGINEERED COMPOSITES OF
WOOD FIBER, MAN-MADE FIBERS AND POLYMERS. NO STRAW WATTLES ARE ALLOWED. Maintenance of Fiber Filtration Tubes

properly maintained. By definition, these devices are designed to provide filtration and it is

�Smooth the soil surface and install the new FFTs following the published installation
