INTRODUCTION
This document, prepared by Engineering and Environmental Services (EES),
contains information relating to the installation of two monitoring wells at the
wastewater treatment facility of Huffman Finishing Plant, Granite Falls, North
Carolina. These wells were installed to monitor the quality of the groundwater
upgradient and downgradient of the wastewater treatment plant. MW-2 was
installed upgradient of the plant and MW-3 was installed downgradient of the
plant. One additional monitoring well unrelated to the wastewater treatment
plant, MW-1 was installed on the same day. The water elevations in the three
wells were used to calculate the flow direction indicated on Drawing Number 91-
075-0005.

MONITORING WELL INSTALLATIONS
Monitoring well MW-2 was installed to a depth of 29 feet. A 10 foot section of
.010 inch slotted 2 inch diameter PVC tubing was placed in the bottom of the well
with 2 inch diameter solid PVC casing completing the installation to the top of the
well. Graded filtration sand was placed around the slotted section to an elevation
2 feet above the top of the slotted section. Two feet of bentonite was poured on
top of the sand and hydrated. The balance of the boring was filled with cement
and grouted around the lockable casing. A 2 foot by 2 foot cement base was set
around the casing of the well and a well identification tag was attached. A
column of water of 14 feet - 2 inches was measured after development and
recovery of the well. The top of the screen was therefore 4 feet - 10 inches
below the surface of the water table. The water level in MW-2 was measured to
be 7.181 feet above the measured water level in monitoring well MW-3 which
was found to exhibit the lowest final water level of the wells installed. The soil
throughout the boring was noted to be a weathered saprolite.
Monitoring well MW-3 was installed to a depth of 15 feet - 2 inches. A 10 foot

encountered at an approximate depth of 14 feet and continued to the final depth.
Well locations are indicated on Drawing No. 91-075-0005 and analytical results
are summarized in a following section and included in the Appendix along with
completed Chains of Custody.
ANALYTICAL RESULTS
Monitoring wells MW-2 and MW-3 were developed, allowed to recover, and
sampled. The samples were analyzed according to the requirements of the North
Carolina Department of Environment Health and Natural Resources
(NCDEHNR), Asheville, North Carolina. These results are summarized in the
table below.

unacceptable to you, you have the right to an adjudicatory hearing before a
hearing officer upon written demand to the Director within 30 days following
receipt of this Permit, identifying the specific issues to be contended. Unless
such demand is made, this Permit shall be final and binding.
This Permit will be effective for the construction of monitor wells from
the date of its issuance until the site assessment has been completed, and shall
be subject to the conditions and limitations as specified therein. Attached are
Well Construction Records to be completed and signed by the drilling contractor.
These forms should be submitted to the Department upon completion of the wells.

the application
dated October
4, 1991, and
in conformity with specifications and
supporting
data, all of which
are filed
with the Department of Environment, Health, and Natural Resources and are
considered a part of this Permit.
This Permit is for well construction only, and does not waive any
provisions or requirements of any other applicable laws or regulations.
Construction of
a well under this Permit shall be in compliance
with
the
North Carolina Well
Construction Regulations and Standards, and any
other
State
and local laws and regulations pertaining to well construction.
This Permit will be effective for the construction of monitor wells from
the
date of its issuance until
the site assessment has
been completed,
and shall
be
subject to other specified
conditions, limitations, or
exceptions as
follows:
1. The well driller shall notify the Asheville Regional Office prior to
actual construction of the wells.

2. The wells shall be constructed with well casing and well screens that
conform to requirements of the "Criteria and Standards Applicable to
Water Supply and Certain Other Type Wells" (15A NCAC 2C .0100).
3. All wells shall be afforded reasonable protection against damage
during construction and use.
4. The well screens shall be set of sufficient depth so there is water
adequate for sampling during the seasonally low water table.
5. Provisions shall be made on the well covers to prevent surface water
with possible contaminants from entering the well pits and
contaminating the monitor wells.
6. Each well shall have permanently affixed an identification plate
constructed of durable material and shall contain the following
information: drilling contractor's name and registration number;
date well completed; total depth of well; a warning that the well
is not for water supply and that the groundwater may contain
hazardous materials; and depth to screens.
7. Well completion, well abandonment, and water quality reports are to be
submitted to the Division of Environmental Management's Groundwater
Section Central Office in Raleigh.
8. When the wells have served their purpose and are of no further use,
they shall be permanently abandoned according to the requirements of
Section .0113 of the "Criteria and Standards Applicable To Water
Supply and Certain Other Type Wells" (15A NCAC 2C).
9. If additional monitor wells, not shown on the location diagram,
need to be constructed, a map showing the proposed location shall be
submitted to the Asheville Regional Office.
