On February 16, 2011 the Division of Water Quality (DWQ) received your information for the above
referenced project. On February 21, 2011 a verbal request was made to you to submit four (4) more PCN
application sets and associated maps. On February 23, 2011 DWQ received a check and maps but no
PCN sets. The submission was incomplete and therefore returned until a complete package can be
submitted.
Additional Information Requested:
® The 401 Certification cannot be processed until five (5) complete sets of the application and
associated maps are received at the DWQ Central Office in Raleigh along with the appropriate
fee.
® The signature on page 10 of 10 (Mark Arcuri) is not valid without an authorization letter from
the owner. Please supply one (1) original and four (4) copies.
Please submit this information within 30 calendar days of the date of this letter. This letter only addresses
(lie application review and does not authorize any impacts to wetlands, waters or protected buffers.
Please be aware that any impacts requested within your application are not authorized (at this time) by the
