Dear Mr. Charles,
I am writing in response to the Notice of Violation letter dated November 20, 2020. I was
under the impression that I had to take corrective actions before submitting the NPDES
Permit Ownership Change Form; I apologize for this oversight on my part.
1. As requested, I have attached the NPDES Permit Ownership Change Form.
2. I have been using the service of the company Clean Septic Tank Service yearly
for the past three years to clean the septic tank. Their last service was rendered on
12/10/2020. After this service, I requested the Water Works,Inc. of Durham to
inspect the septic tank to take any further corrective actions if needed. A copy of
their NCOWCICB Certified Septic Inspection Report completed on 12/11/2020 is
attached. This report certifies that there is no evidence of tank leakage (seepage).
3. I have purchased chlorine tablets and placed them in the chlorinator as instructed
by Ms. Jane Bernard during her visit on September 2, 2020. I have attached a
copy of the receipt of the purchase of the approved chlorine tablets.
4. I have requested Mr. Chad from Don wells to collect and analyze a sample of
effluent leaving the treatment system prior to discharge. However, he was unable
to collect a representative sample of the effluent discharge during his visit due to
insufficient flow from the discharge pipe. I arranged for him to come and collect a
sample for testing at a later date, assuming conditions for sampling are favorable.
As soon as the report is available, I will promptly send it to your office.

Once again, my sincere apologies for the oversight. As requested, I will continue to
inspect the system to ensure that the treatment components are properly maintained.
