The following is an abbreviated application form for those applying for a 401 Water Quality
Certification to impact FERC (Federal Energy Regulatory Commission) regulated lake shore or
bottom under the U.S. Army Corps of Engineers Permit GP30. This Application cannot be used
for wetland or stream impacts. Please provide five (5) copies of this application and supporting
information as well as a non-refundable application fee to the Division of Water Quality of $240.00
for impacts to lake bottom of less than 1 acre and $570.00 for impacts to lake bottom of greater or
equal to 1 acre.

Description of Planned Excavation:
Proposed excavation is for Lot 15 shown on the map. The development is Sunset Pointe, and the reservoir is High Rock
Lake. Contour lines are shown at 2 foot intervals. Proposed excavation starts at the dotted red line and tapers down to a
depth of 8 feet below full pond. The area will be approximately 50 deep x 50 feet wide totaling 2500 scl ft, and about
300 cu yds of excavated material, to accommodate ample room around a floating dock.
Method of Excavation and Plan for Disposal of Excavated Material:
A licensed contracted grader will perform the excavation using small grading equipment such as a skid steer with a front
loader. Excavated material will be removed by trailer to a location away from the property.
Schedule of Conducting Proposed Excavation:
Excavation and cleanup will be completed in less than one day. Excavation is planned while High Rock Lake is lowered for
such purposes, up to the date of 2/21/2011.
