This Application cannot be used for wetland or stream impacts. Please provide five (5) copies of this application and supporting information as well as a non-refundable application fee to the Division of Water Quality of $240.00 for impacts to lake bottom of less than 1 acre and $570.00 for impacts to lake bottom of greater or equal to 1 acre. Application for Excavation Applicant: Michael Wetzel Property Address: 425 Sunset Pointe Dr, Salisbury, NC 28146 Map of Area of Reservoir and Shoreline where Excavation is Proposed: ® Long Ferry Rd Description of Planned Excavation: Proposed excavation is for Lot 15 shown on the map. The development is Sunset Pointe, and the reservoir is High Rock Lake. Contour lines are shown at 2 foot intervals. Proposed excavation starts at the dotted red line and tapers down to a depth of 8 feet below full pond. The area will be approximately 50 deep x 50 feet wide totaling 2500 scl ft, and about 300 cu yds of excavated material, to accommodate ample room around a floating dock. Method of Excavation and Plan for Disposal of Excavated Material: A licensed contracted grader will perform the excavation using small grading equipment such as a skid steer with a front loader. Excavated material will be removed by trailer to a location away from the property. Schedule of Conducting Proposed Excavation: Excavation and cleanup will be completed in less than one day. Excavation is planned while High Rock Lake is lowered for such purposes, up to the date of 2/21/2011.
