November 5, 2003 Davidson County DWQ Project No. 03-1102 CERTIFIED MAIL: 7002 2030 0002 7223 7380 RETURN RECEIPT REQUESTED WETLANDS 1401 GROUP Mr. Charles Veach 7415 High Pines Church Road Asheboro, NC, 27203 Dear Mr. Veach: NOV 13 2003 MATER QUALITY SECTION On September 19, 2003, the NC Division of Water Quality (DWQ) received your application for a General 401 Water Quality Certification to impact 143 linear feet of stream in order to construct the proposed Mallard Creek Subdivision in Wallburg, Davidson County. In accordance with 15A NCAC 2H .0506, an inspection of the site was conducted on October 29, 2003 by Mr. Daryl Lamb of the Division of Water Quality (DWQ), Winston-Salem Regional Office. In your application, a road crossing consisting of a double pipe culvert was listed as the only impact on State waters that would occur as a result of this project. Since Mr. Lamb's site visit, your consultant, Westcott Engineering & Consulting, PC, has informed the DWQ that additional stream impacts may occur as a result of this project. These additional impacts are not listed in your current 401 application. In order that we may determine your project's compliance with 15A NCAC 2H .0500, please provide seven copies of the following information: 1. A complete and correct General 401 Water Quality Certification application (Pre-Construction Notification Application Form) listing and fully describing all impacts to State waters that will occur as a result of your proposed project. You should include site maps that illustrate these impacts with respect to the planned building lots. In your reply, please refer to the DWQ number listed above. Until the information requested in this letter is provided, I will request (by copy of this letter) that the U.S. Army Corps of Engineers place this project on hold. Also, this project will be placed on hold for DWQ processing due to incomplete information (15A NCAC 2H.0507(h)). The processing of this application will begin when the requested information is received. Please send the requested information to Ms. Cyndi Karoly, N. C.
