' To Whom It May Concern:
This submittal is a Pre-Construction Notification (PCN) for a project at 2677 Knob
Hill Drive (a vacant lot) in Clemmons, to remove existing stormwater culverts and
restore a stream on an un-named tributary of Muddy Creek. All construction
associated with the project will be contained within the property limits and storm
drainage easement owned by the Village of Clemmons. Please find enclosed the
following information and data for this project:

The project is located within an established residential area. The site is downstream of
an existing, undersized, stormwater drainage system under Knob Hill and Boyer
Drives which are anticipate to be upgraded in the future.
' The project includes: 1 restoration of a stream reach that was formerly by-passed by
several concrete pipes which were improperly placed, 2) removal of three sections of
66" RCP at the current outlet and replacement with a stabilized discharge outlet in the
form of a scour basin with discharge weir, and 3) installation of a headwall and wing
walls at the new outlet location.

application:
Project site is an empty lot in a residential area. Stormwater runoff discharges through the property to the northeast
corner from surface runoff, a 66-inch RCP under Knob Hill Drive from the west, and a 30-inch CMP from south of the
property. Two runs of 44-inch RCP (in series) were formerly placed to divert primary flow to the north side of the property
in an apparent attempt to abandon a former channel to make the property suitable for development. With approval from
NC DWQ, a temporary relief channel was recently excavated from the existing 66-inch discharge to the primary discharge
channel at the northeast corner.

To restore a formerly filled natural channel and to reduce upstream flooding by removing flow constrictions on the site.

Remove a portion of the existing 66-inch and 30-inch pipes, temporarily divert flow through the existing 44-inch pipes,
construct a headwall and stilling basin to receive flow from the 66-inch and 30-inch pipes, construct a new natural channel
from the stilling basin to the natural stream, divert flow to the new channel system, remove the 44-inch pipes, grade and
stabilize the site. Work will be performed with a trackhoe, backhoe, skid-steer, dumptruck and similar equipment.
