Please provide a completed Level Spreader-Vegetated Filter Strip Supplement
Form, with all required items, available at:
http://portal.ncdenr.org/web/wq/ws/su/bmp-manual. Failure to provide all of the
required items (including the calculations and the signed and notarized Operation
and Maintenance Agreement) will result in a return of application. All plan sheets
must be at the required scales (or larger) and topographic contours must be
provided between the level spreader and the receiving stream.
2. The current design of the level spreader fails to meet the requirements of Chapter
8 of the BMP Manual (available at the above web site). The level spreader has
been placed upslope of the dissipator pad for the pond outlet. Also, the slopes
must be appropriate for the type of vegetated filter strip that is proposed.
