Stormwater Permit NCGO10000 for Construction Activities has not been paid. The annual fee was billed on 11/24/2020 and was
due on 12/24/2020.

If the project was closed out prior to the invoice date, you must submit a Notice of Termination on-line, and DEQ will determine if
waiving the fee is appropriate. Failure to address this deficiency within 30 calendar days could result in the issuance of a Notice
of Violation and potential civil penalty assessment of up to $25,000 per day for failing to maintain permit coverage. Should you
