A Fast Track Sewer System Extension Application for the Leland Highway 17 - Sewer Expansion II — Phase 2 project dated October 20", 2020 and a check for the application fee were delivered via Fed Ex on December 4", 2020 to the North Carolina Department of Environmental Quality (NCDEQ) Wilmington Regional Office. The original Fast Track Sewer System Extension Application was signed and sealed by the project manager/engineer of record at the time, Mr. Richard J. Rudin, and was delivered electronically to your attention on November 11"', 2020. Mr. Richard J. Rudin has departed from our firm and will no longer be the engineer of record who will certify the project at the conclusion of construction. I, J. Clark Maness, attest that the submitted Fast Track Sewer Extension Application has been reviewed by me and is accurate, complete and consistent with the information supplied in the plans, specifications, engineering calculations, and all other supporting documentation to the best of my knowledge. I further attest that to the best of my knowledge the proposed design has been prepared in accordance with the applicable regulations, Minimum Design Criteria for Gravity Sewers (latest version), and the Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and Force Mains (latest version). This project will be connecting to the Phase 1 Project just completed under Permit No. WQ0040750. There will be no flow requested with this application. The flow will be requested as development progresses in the area and then each new structure/development will apply for capacity through subsequent permits. The Leland Highway 17 — Sewer Expansion II — Phase 2 Project consists of approximately 155 LF of 10- inch gravity sewer pipe that will connect to a new duplex pre -cast submersible pump station (capability of becoming a triplex in the future) along Old Town Creek Road with approximately 7,129 If of parallel 10-inch (immediate service) and 16-inch (future service) forcemains. The forcemains will travel along Old Town Creek Road, under US 17 by horizontal directional drill and then heads north along Hazels Branch Road to connect with existing 10-inch and 16-inch force mains mentioned above. The sizing of the 10-inch force main is based on the initial projected pumping capacity of 1.0 mgd based on information provided by the developers engineer who will be required to request capacity as the development progresses as mentioned above. The sizing of the 16-inch force main is based on the projected future growth in this area and also further growth South along Highway 17 where the projected future pumping capacity is approximately 2.0 mgd.
