TO DISCHARGE WASTEWATER UNDER THE
NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provisions of North Carolina General Statute 143-215.1,
other lawful standards and regulations promulgated and adopted by the North Carolina
Environmental Management Commission, and the Federal Water Pollution Control Act, as
amended,

SUPPLEMENT TO PERMIT COVER SHEET
Mid South Water Systems, Inc.
is hereby authorized to:
1. Enter into a contract for construction of a wastewater treatment facility,
and
2. Make an outlet into an unnamed tributary to the Catawba River, and
3. After receiving an Authorization to Construct from the Division of
Environmental Management, construct and operate a 0.250 MGD wastewater
treatment facility located off Turnberry Lane, north of the intersection of
Highway 73 and NCSR 1395, west of Cowans Ford Dam in Lincoln County (See
Part III of this Permit), and
4. Discharge from said treatment works at the location specified on the
attached map into an unnamed tributary to the Catawba River which is
classified Class "WS-III" waters in the Catawba River Basin.

F. Toxicity Reopener
This permit shall be modified, or revoked and reissued to incorporate toxicity
limitations and monitoring requirements in the event toxicity testing or other
studies conducted on the effluent or receiving stream indicate that detrimental
effects may be expected in the receiving stream as a result of this discharge.
G• If this facility is built in phases, plans and specifications for the next
phase shall be submitted when the flow to the existing units reaches 80% of the
design capacity of the facilities on line. At no time may the flow tributary
to the facility exceed the design capacity of the existing units.
