In compliance with the provisions of North Carolina General
Statute 143-215.1, other lawful standards and regulations promulgated
and adopted by the North Carolina Environmental Management Commission,
and the Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge wastewater from a facility located

off of Highway 16
Lincoln County
to receiving waters designated as Lucky Creek (Lake Norman) in the
Catawba River Basin
in accordance with effluent limitations, monitoring requirements, and
other conditions set forth in Parts I, II, and III hereof.

SUPPLEMENT TO PERMIT COVER SHEET
Mr. David Seaford
is hereby authorized to:
1. Enter into a contract for construction of a wastewater
treatment facility, and
2. Make an outlet into Lucky Creek (Lake Norman), and
3. After receiving an Authorization to Construct from the
Division of Environmental Management, construct and operate a
0.045 MGD package type wastewater treatment facility located
off of Highway 16 in Lincoln County (See Part III, condition
No. B. of this permit), and
4. Discharge from said treatment works into Lucky Creek (Lake
Norman) which is classified Class "A —II and B" waters in the
Catawba River Basin

Lincoln County, travel north on Highway 16 approximately 1.4 miles and
turn right on a paved access road. Travel approximately 0.3 miles to
the end of the road. The proposed site is on the left side of the road,
facing the lake.
6. a. The coordinates to the proposed goint of effluent discharge are:
Latitude: 35028'05" Longitude: 80 59'15"

7. Size (land available for expansion and upgrading): There is ample area
available for the construction of the proposed wastewater treatment
facilities.
