Subject: Request for a NW 3 Permit and DWQ 401 for the replacement of Bridge # 6 over Ready
Branch on US 17 in Martin County.
The North Carolina Department of Transportation is requesting the issuance of a NW 3 permit
and corresponding DWQ certification for the replacement of bridge # 6 in Martin County. The bridge is
located on US 17 approximately 0.1 miles north of the intersection of SR 1521 (Mill Inn Rd). The bridge
-spans Ready _Branch,_which.is-classified.by- DWQ.as.class-`C"-waters.-Bridge #_ 6_ is. currently-a.concrete----
structure that is 90 ft. long by 28 ft. wide. This structure will be removed using NCDOT's guidelines for
bridge removal. Coffer dams will be utilized around the interior bent and southern end bent during removal.
The proposed bridge will be replaced on its existing alignment. The new bridge structure will be 145 ft. in
length by 40 ft. wide and built with precast cored slab concrete deck, support piles and bents. The bridge
roadway approaches will have to be reconstructed. An on-site detour will be used utilizing the existing
bridge on the north bound lanes of US 17. There is a 12" gas main that will be relocated to the west side of
US 17 and will be installed under Ready Branch Creek using a trenchless method and will not result in any
additional impacts. This project will be funded through federal stimulus money.
Included with this letter are the location map, permit drawings, and PCN. If you have questions
please call me at (252) 482-7977.

The existing concrete bridge deck and interior bent will be removed in accordance with NCDOT guidelines for bridge
demolition and removal. The interior bent has a concrete footing and a coffer dam will be needed to encompass the area
during removal. A coffer dam will also be needed for the removal of the southern end bent. This will result in 0.01 acres
of temporary surface water impacts. The new bridge will be 145 feet in length by 40 feet wide. The new bridge will be a
three span cored slab concrete structure supported by concrete piles. The new roadway approaches will be widened to
accommodate the bridge structure. Equipment used during this project will include a crane, excavator, grading
