SUBJECT: Bridge #58 Replacement on SR 1405 (Jolly Cemetery Road), Alexander County
Dear Ms. Hair:
The North Carolina Department of Transportation requests authorization to replace an existing
bridge on SR 1405 (Jolly Cemetery Road) in Alexander County. NCDOT proposes to replace an
existing timber deck bridge with a 50-foot long, 28.7' x 93 aluminum box culvert with
associated riprap for structure stabilization. A knee-wall set to bankfull height will be
constructed in the center of the structure to maintain "normal" channel width inside the culvert.
The knee-wall is also being constructed in an effort to initiate natural creation of a bankfull
bench within the wide culvert during bankfull flows. Minor stream work will be necessary-at the
inlet to align the stream with the mainflow side of the culvert. A 50-floodplain bench will be
constructed at the culvert inlet which will extend from the kneewall to existing contours to allow
for bankfull flow events to access both sides of the culvert. The bank at the culvert outlet will
be excavated to bankfull elevation, which will also allow bankfull flow events to flow through
the culvert and back into the channel without causing additional bank erosion. A temporary
cofferdam pumping system will be used to divert the stream flow around the construction sites as
needed. Areas affected by temporary cofferdam system will be restored back to pre-project
conditions: The scope of the work will result in 110 feet of permanent impacts (50 feet of culvert
fill, 50 feet of upstream floodplain bench construction, 10 feet of riprap impacts at outlet) to
Muddy Fork Creek, which is classified as Class C waters by the Division of Water Quality.
The project area is not located within '/ mile of the critical watershed area nor is SR 1405 a rural
or urban arterial; therefore, hazardous spill basins are not required. Enclosed is the hazardous
spill basin checklist.
This office has conducted a GIS and field survey for the project for historical sites and
threatened/endangered species sites. No sites listed on the National Register of Historic Places
are located in the vicinity of the project nor did there appear to be any eligible structures. The
federally listed species for Iredell County (USFWS listed date September 22, 2010) include the
Dwarf-flowered heartleaf (Hexastylis nanrflora) and the Bog Turtle (Clenunys rnuhlenbergii).

The Natural Heritage Program records document the nearest dwarf-flowered heartleaf location
approximately 5 miles east of the proposed project. The heartleaf is usually found on dry to
moist ravine slopes (usually north facing and of the oak-hickory-pine forest type) and in boggy
areas near springheads. There is no potential habitat within the proposed project area, and no
species were observed. National Heritage Program records document the nearest bog turtle
location approximately 10.5 northwest of the proposed project area. Bog turtles are usually
found in bogs, marshes, and wet pastures. There is no suitable habitat located within the project
area. The proposed project will have a finding of "no effect" on the dwarf-flowered heartleaf
(Hexastylis iraiaiJlora) species'6r bog turtle (Clemnrys muldenbergii).

To replace a deteriorating timber deck bridge for the safety of the traveling public.

NCDOT proposes to replace a timber deck bridge with a 50-foot long, 28.7 ft x 9.3 ft aluminum box culvert with
associated riprap for structure stabilization. A "kneewall" will be constructed inside the aluminum box culvert. The
kneewall height will be at bankfull elevation. Fifty foot long floodplain benches will be constructed upstream and
downstream of the high flow culvert cell.
