REQUEST FOR MORE INFORMATION
Dear Sir or Madam:
On October 22, 2010, the Division of Water Quality (DWQ) received your application dated October 22,
2010, to fill or otherwise impact 0.35 acres of 404/wetland (0.293 acres permanent impact and 0.057 acres
temporary impact), and 192 linear feet of perennial stream (148 linear feet permanent impact and 44 linear
feet temporary impact) and 148 linear feet intermittent stream impact (121 linear permanent impact and 27
linear feet temporary impact) associated with construction of the proposed Salem Village subdivision and
the segment of Apex Peakway from Tingen Road to Salem Street at the site. On November 5, 2010, the
DWQ received more information, however, the DWQ has determined that your application remains
incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional
information in order to process your application to impact protected wetlands and/or streams on the subject
property. Therefore, unless we receive five copies of the additional information requested below, we will
place this project on hold as incomplete until we receive this additional information. If we do not receive
the requested information, your project will be formally returned as incomplete. Please provide the
following information so that we may continue to review your project.
Additional Information Requested:
Please address the following issues with regard to the conceptual stormwater management plan:
a. BMP 1 is proposed to treat portions of Phases 2A and 3. Please provide evidence that the
BMP is adequately sized to handle the flow from both of these two phases. Also, include
a note on the plans that states that if the two phases are not constructed simultaneously,
then there will be adequate Sediment and Erosion Control Devices to treat runoff from
the later phase prior to stormwater being discharged to BMP 1.
b. It appears that a portion of the drainage area to BMP 1 is at a lower elevation than the
BMP itself.
c. The delineation of the drainage areas does not appear to follow contour lines.
d. Significant portions of Phases 3 or 4 are not included in the drainage areas of any of the
conceptual BMPs. Please either rework the drainage areas to include these unaccounted
for areas or provide an explanation as to why those areas were omitted.

requested information within 30 calendar days of the date of this letter, your project will be withdrawn and
you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized (at
