There is a typo on our responsible owner portion of our issued permit for NCG060373. The zip code and city of
Westfield, WI that was submitted for the ownership change application is correct in the application, but the summary
report that I can pull up online for my client has Pickett, WI and a zip code that is 1 digit off listed as
54964 instead of the correct zip code of 53964 for Westfield, WI.
The correct city for the person "legally responsible for the permit" is Westfield, WI 53964.
What do we need to do to correct this typo in the permit?
