Subject: Application for a CAMA General Permit for utility work associated with the
replacement of Bridge No. 19 on eastbound US 158 over the Pasquotank River in
Pasquotank and Camden Counties. Federal Aid Project No. STP-0158(31) TIP
No. U-4438, Debit $400.00 from WBS Element 35742.1.1.
Please find enclosed the North Carolina Division of Coastal Management Major Permit Form
and utility plans for the above referenced project. An Environmental Assessment and Finding of
No Significant Impact were completed for this project on April 29, 2009 and January 28, 2010
respectively and distributed shortly thereafter. Additional copies are available upon request.
The North Carolina Department of Transportation (NCDOT), Division of Highways, in
consultation with the Federal Highway Administration (FHWA), proposes improvements to US
158 from NC 34 (Water St.) to US 17 Business (Road St.) including the replacement of the
existing 846-foot Bridge No. 19 with new bridge of the same length over the Pasquotank River
in Pasquotank and Camden Counties. No impacts to wetlands or surface waters due to utilities
are proposed. This application is for utility work associated with the project and is being
requested in advance to allow time for utility relocations to commence prior to project letting.
The proposed let date for the project is April 19, 2011 with a review date of March 1, 2011.
However, the let date may advance as additional funds become available.
Regulatory Approvals
Section 404 Permit: A request for the appropriate Section 404 nationwide permit from the U.S
Army Corps of Engineers will be submitted under separate cover.

Section 401 Permit: A request for the appropriate Section 401 General Certification from the
North Carolina Department of Environmental and Natural Resources, Division of Water Quality
will be submitted under separate cover.
CAMA: NCDOT requests that the proposed utility work associated with this project be
authorized under a Coastal Area Management Act General Permit. The remaining construction
activities will be requested via a Coastal Area Management Act Major Development Permit. The
adjacent riparian landowner return receipts will be forwarded upon receipt. Authorization to
debit the $400 Permit Application Fee from WBS Element 35742.1.1 is hereby given.
A copy of this permit application will be posted on the NCDOT Website at:
