The Mooresville Regional Office (MRO) received and reviewed the 2014 Annual Report for the
above permit. The data submitted appears to reflect compliance with permit requirements.
This permit was rescinded on July 28th, 2014 and the facility added as a source to the Two Rivers
Utilities permit # WQ0001793; however, should you have questions about either permit please

Dear Sir or Madam.:
During the first half of 2014, the Town of Stanley held Land Application of Residual Solids (503) permit
#WQ0002081 for the land application of residual solids from its Lola Street Wastewater Treatment Plant
(WWTP). Under this permit during 2014, a total of 8.23 dry tons of class-B liquid biosolids from the Lola
Street WWTP was applied to 21.4 acres of permitted farmland in North Carolina. No compliance issues
were identified with the treatment or application of these biosolids.
The Town of Stanley is proceeding towards a wastewater connection with the City of Gastonia's Two
Rivers Utilities system and the decommissioning of the Lola Street WWTP. Effective June 26, 2014, a
permit modification included the Lola Street WWTP as a residual source in Two Rivers Utilities' Land
Application of Residual Solids (503) permit #WQ0001793. The Town of Stanley's permit #WQ0002091
was then rescinded. Accordingly, reporting regarding biosolids from the Lola Street WWTP applied after
June 26, 2014 will be included in the City of Gastonia's annual report for permit #WQ0001793.
Attached are three copies of the required annual report on Town of Stanley's land application
activities that took place in North Carolina under permit WQ0002081 during the 2014 calendar year.
A copy of this report is also being submitted to the United States Environmental Protection Agency as
required.
The Town of Stanley prepared the biosolids through aerobic digestion at the Lola Street WWTP (NPDES
Permit #NC0020036). This facility is located at 109 Lola Street, Stanley, NC 28164.
I serve as permittee for the Lola Street Lift Station's NPDES permit and served as permittee for Land
Application of Residual Solids (503) permit #WQ0002081. Please feel free to contact me with any

During this period, Synagro provided contract land application services for the Town of Stanley.

PART B
Facility was compliant during calendar year 2014 with all conditions of the land application permit (including but not
limited to items 1-12 below) issued by the Division of Water Quality X YES NO.
If no, Please provide a written description why the Facility was not compliant, the dates, and explain corrective action
taken.
1. Only residuals approved for this permit were applied to the permitted sites.
2. Soil pH was adjusted as specified in the permit and Tune was applied (if needed) to achieve a soil pH of at least
6.0 or the limit specified in the permit.
3. Annual soils analysis was performed on each site receiving residuals during the past calendar year and three (3)
copies of laboratory results are attached.
4. Annual TCLP analysis was perfonmed and three (3) copies of certified laboratory results are attached.
5. All other monitoring was performed in accordance with the permit and reported during the year as required and
three (3) copies of certified laboratory results are attached.
6. The facility did not exceed any of the Pollutant Concentration Limits in Table 1 of 40 CFR part 503.13 or the
Pollutant Loading Rates hi Table 2 of 40 CFR part 503.13 (Applicable to 40 CFR Part 503 regulated
facilities).
7. All general requirements in 40 CFR Part 503.12 and management practices in 40 CFR Part 503.14 were
complied with (Applicable to 40 CFR Part 503 regulated facilities).
8. All operations and maintenance requirements in the permit were complied with or, in the case of a deviation,
prior authorization was received from the Division of Water Quality.
9. No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of
violations are attached to include appropriate actions and remediations.
10. - Vegetative cover as specified in the permit was maintained on this site and crops grown were removed in
accordance with the crop management plan.
11. No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred.
12. All buffer requirements as specified on the permit were maintained during each application of residuals.

Understanding the Soil Report: explanation of measurements, abbreviations and units
Recommendations
Lime
If testing finds that soil pH is too low for the crop(s) indicated, a lime recommendation will be given in units of either
ton/acre or Ib/1000 sq ft. For best results, mix the lime into the top 6 to 8 inches of soil several months before planting.
For no -till or established plantings where this is not possible, apply no more than 1 to 1.5 ton/acre (50 Ib/1000 sq ft) at
one time, even if the report recommends more. You can apply the rest in similar increments every six months until the
full rate is applied. If MG is recommended and lime is needed, use dolomitric lime.
Fertilizer
Recommendations for field crops or other large areas are listed separately for each nutrient to be added (in units of
lb/acre unless otherwise specified). Recommendations for N (and sometimes for B) are based on research/field'studies
for the crop being grown, not on soil test results. K-1 and P-1 values are based on test results and should be > 50. If they
are not, follow the fertilizer recommendations given. If Mg is needed and no lime is recommended, 0-0-22 (11.5% Mg) is
an excellent source; 175 to 250 lb per acre alone or in a fertilizer blend will usually satisfy crop needs, SS-1 levels appear
only on reports for greenhouse soil or problem samples.
Farmers and other commercial producers should pay special attention to micronutrient levels. If $, pH$, $pH, C or Z
notations appear on the soil report, refer to $Note: Secondary Nutrients and Micronutrients. In general, homeowners do not
need to be concerned about micronutrients. Various crop notes also address lime fertilizer needs; visit
ncagr.gov/agronomi/pubs.htm.
Recommendations for small areas, such as home lawns/gardens, are listed in units of Ib/1000 sq ft . If you cannot
find the exact fertilizer grade recommended on the report, visit www.ncagrgov/agronomi/obpart4.htm#fs to find

Dear Mr. Jenkins:
The Mooresville Regional Office (MRO) conducted a compliance inspection on May 2"d, 2014,
which included a land application event and facility visit. The annual report reflected there was no
land application in 2013. .
This permit is due to expire on August 31, 2014, which means that a renewal application was due
in February; however, per conversation with John Woods, the Town of Stanley is working with Two
Rivers Utilities (TRU-Gastonia) to assist with closure of Stanley's WWTP facility. The MRO spoke
with David Shellenbarger (TRU) on May 14th, 2014; who, further elaborated that he was submitting
a modification to their permit #W00001793 to add. the Town of Stanley as a residuals source, at
which point a request for rescission will be submitted for this permit #W00002081.

PART B
Facility was compliant during calendar year 2012 with all conditions of the land application permit (including but not
limited to items 1-12 below) issued by the Division of Water Quality X YES NO.
If no, Please provide a written description why the Facility was not compliant, the dates, and explain corrective action
taken.
1. Only residuals approved for this pen -nit were applied to the permitted sites.
2. Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at least
6.0 or the limit specified in the permit.
3. Annual soils analysis was performed on each site receiving residuals during the past calendar year and three (3)
copies of laboratory results are attached.
4. Annual TCLP analysis was performed and three (3) copies of certified laboratory results are attached.
5. All other monitoring was performed in accordance with the permit and reported during the year as required and
three (3) copies of certified laboratory results are attached.
6. The facility did not exceed any of the Pollutant Concentration Limits in Table 1 of 40 CFR part 503.13 or the
Pollutant Loading Rates in Table 2 of 40 CFR part 503.13 (Applicable to 40 CFR Part 503 regulated
facilities).
7. All general requirements in 40 CFR Part 503.12 and management practices in 40 CFR Part 503.14 were
complied with (Applicable to 40 CFR Part 503 regulated facilities).
8. All operations and maintenance requirements in the permit were complied with or, in the case of a deviation,
prior authorization was received from the Division of Water Quality.
9. No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of
violations are attached to include appropriate actions and remediations.
10. Vegetative cover as specified in the permit was maintained on this site and crops grown were removed in
accordance with the crop management plan.
11. No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred.
12. All buffer requirements as specified on the permit were maintained during each application of residuals.
