Division of Water Resources' Central and Regional staff has reviewed the application package
received October 20, 2020. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on January
31, 2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through F, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

F. Residuals Source Certification (FORM: RSC 06-16):
The process flow diagram for the South Fork Industries — Maiden Plan indicates that the residuals
are generated from a biological activated sludge process, which would categorize them as
biological residual in accordance with 15A NCAC 02T .1102(6). Class A biological residuals are
subject to the pathogen reduction requirements of 15A NCAC 02T .1106(a) and the vector
attraction reduction requirements in 15A NCAC 02T .1107, unless it can be demonstrated that they
are pathogen free in accordance with the exemptions in 15A NCAC 02T . 1106(b) and 15A NCAC
02T .1107(c).
Please specify how residuals generated at the permitted facility will meet pathogen and vector
attraction reduction requirements listed in Section II.7. and II.8. of the RSC 06-16 form in
accordance with 15A NCAC 02T .1106 and .1107. Alternatively, provide evidence as to why the
facility would be exempt from providing these requirements through 15A NCAC 02T .1106(b) and
15A NCAC 02T .1107(c). This would entail providing documentation that the residuals are
generated from industrial wastewater only (i.e. no domestic contributions), and are pathogen free
(i.e. meeting Class A standards through density of fecal coliform or Salmonella sp. bacteria).
