As agents for the City of High Point, we are enclosing five copies of the Amended Pre-Construction
Notification package (pages 11 and 12) in application fora 401 Water Quality Certificate for the High
Point's City Lake Raw Water System Transmission Main Improvements project. This amends the
Notification Package forwarded to you with cover letter dated Apri15, 2006.
During a meeting with Daryl Lamb on May 31, 2006, it was determined that a potential allowable
impact of Zone 2 (Randleman Rule) required this amended application.
This package includes the following:
• Amended PCN Application Form (pages 11 and 12)
• A figure detailing the impact area near station 91+00.
