REQUEST FOR ADDITIONAL INFORMATION
Dear Mr. Jones:
On October 7, 2010, the Division of Water Quality (DWQ) received your Permit application
dated June 5, 2010 regarding your request to install 80 linear feet of 96 inch corrugated metal
pipe (CMP) for a road crossing on Parker Creek. The DWQ has determined that your
application is incomplete and/or provided inaccurate information as discussed below.
The DWQ will require additional information in order to process your application to
impact protected wetland, streams and/or buffers on the subject property. Therefore,
unless we receive the additional information requested below, we will have to move
toward denial of your application as required by 15A NCAC 2H .0506 and 213 .0233(8)
and will place this project on hold as incomplete until we receive this additional
information. Please provide the following information so that we may continue to review
your project.
Additional Information Requested:
• Total length of proposed project is inconsistent within application. PCN application
(page 5 of 11) item (3.) Stream Impacts; indicated 100 linear feet of stream impacts.
Plan drawings show 80 linear feet of CMP and 20 feet of riprap at both inlet and
outlet of subject pipe for a total of 120 linear feet of impacts.

• It is required that pipe be placed 1 foot below stream bed elevation to accommodate
aquatic passage during low flow conditions. The location of the bench mark used
for culvert placement should be indicated on plans for future compliance inspection.
Plans should reflect pipe placement elevations.
• It is necessary to minimize impacts in stream channel by reducing foot print of
riprap used in the stream at inlet and outlet of CMP.
• Riprap used for energy dissipation should be placed below stream bed elevation as
so not to interfere with aquatic passage during low flow conditions. Plans should
reflect elevation of riprap placement.
• Buffer impacts for roadway construction should be calculated from toe of fill to toe
of fill. Impacts should be shown for both Zone I and Zone II.
• Location of drop inlets for curb and guttering should be shown on plans. Point of
stormwater discharge should be noted on plans.
Please respond in writing to the DWQ within 30 days of receipt of this letter with the
requested information. If we do not hear from you within 30 days, we will assume that
you no longer want to pursue this project and we will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to
wetlands, waters or protected buffers. Please be aware that any impacts requested within
your application are not authorized (at this time) by the DWQ. Please call Roberto
