For the purpose of this permit, a responsible corporate officer means: (a) a president, secretary, treasurer or vice president of the corporation in charge of a principal business function, or any other person who performs similar policy or decision making functions for the corporation, or (b) the manager of one or more manufacturing production or operating facilities employing more than 250 persons or having gross annual sales or expenditures exceeding 25 million (in second quarter 1980 dollars), if authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures. (2) For a partnership or sole proprietorship: by a general partner or the proprietor, respectively; or (3) For a municipality, state, federal, or other public agency: by either a principal executive officer or ranking elected official. Please note that the facility in question is a new ready mixed concrete manufacturing facility currently nearing the end of construction phases and that manufacturing and operational testing has been occurring since approximately October 16th, 2020. With regards to the enclosed NO1 application package, please find the following materials: • A completed Notice of Intent form, • A complete map set depicting the items outlined within Section 10 of the NO1 form (including 4 sheets from the engineered site plans set used for design and local approval of this plant), • A line drawing depicting the flow of water through this facility as outlined within Section 10 of the NO1 form, • A narrative description of wastewater at this facility (see below), • A check for the application fee (total fee = $100.00), made payable to "NCDEQ" With regards to a narrative description of wastewater at this facility, we offer the following: The discharge from this' facility (from identified, designated outfall PDO001) would be a combined (commingled) stormwater / noh-stormwater discharge from standard, typical ready -mixed concrete plant operations. Any discharge at this facility is first captured and managed through on -site features, which should serve to improve water quality (commingled discharge, in the case of a sand filtration BMP) and to allow for on -site reuse/recycling (in the case of a non-stormwater / process water settling basin system for most concrete plant activities). It is anticipated that under most circumstances, the discharge would be primarily composed of stormwater (only), and may only have a commingled discharge infrequently, such as during unusually large storm events. Any non-stormwater (wastewater) generated from concrete plant operations would occur from concrete mixer truck exterior washing, mixer truck interior washout (which will only be drained into the settling basin system for reuse / recycling), aggregate stockpile watering, plant yard watering, etc.; the large majority of which is intended to be captured in the non-stormwater settling basin system for further reuse / recycling (such as for further washing and further concrete production). Environmental Compliance & Due Diligence Services Regardless of source, any discharge from this facility will first pass through a large, elaborate sand filtration system prior to discharge, significantly enhancing water quality of any final discharge (there is no direct discharge of anything from this site — stormwater or non-stormwater — which does not first pass through this BMP).
