Minor Modification Form, Part A8 —Please add to this item that the LIGSs are only required for local quantity requirements and that the State approved SCM (Filterra) has not been modified (Just for clarity). Form updated as requested. 2. Part B2 — Please provide a second copy of plan sheet C-1.6 (Only one was provided) and 2 hard copies of any of the other plan sheets (that are part of the permit package) that have been changed since having issued the permit. For example, plan sheet C-2.8 has the details of the UGS on it and these will be changed by this minor mod so would need to be replaced). Only applicable plan sheets that were included in the permit package need to be updated (The permittee should have a hard copy of the permit package or if they lost it, there is a hard copy of the permit package at the Raleigh Regional Office). 'The correct number Qfhard copies of applicable sheets are included with this resubmittaL Please provide the following: 3. 2x hard copies of the revised first page of the Minor Modification Application (new signatures are not needed, I'll slip sheet the revised pages in). Two hard copies of the first page are now included. 4. 1x full electronic copy of the revised Minor Modification Application. The revised Minor Modification Application has been ernailed to fim Farkas. 5. 1x hard copy of plan sheet C-1.6. One hard copy of the grading sheet is now included. 6. 2x hard copies of any other applicable plan sheets. Two copies of the revised UGS Detail sheet is now included. 7. Ix electronic copy of the applicable plan sheets. The appikable sheets have been emailed to fien Farkas.
