You are receiving a Notice of Deficiency for failure to pay the annual permit fee per the requirements of N.C. General Statute
143-215.3D(a)(4). You are receiving this notice because the annual fee of $100.00 for coverage under NPDES General
Stormwater Permit NCGO10000 for Construction Activities has not been paid. The annual fee was billed on 11/3/2020 and was
due on 12/3/2020.
The annual permit fee applies to the following construction site:

Certificate of Coverage No. NCC191171
If the project was closed out prior to the invoice date, you must submit a Notice of Termination on-line, and DEQ will determine if
waiving the fee is appropriate. Failure to address this deficiency within 30 calendar days could result in the issuance of a Notice
of Violation and potential civil penalty assessment of up to $25,000 per day for failing to maintain permit coverage. Should you
