outfalls are maintained for emergency bypass procedures. Discharges are released at the
tailrace of the dam. Oil/water separators are used on Outfalls 001,004, 007. 010.and 013
to remove oil that might leak from the generators or associated equipment. Outfall 014
discharges groundwater seepage through the dam to ensure structural integrity.

Comment: Duke Energy Carolinas, LLC has been certified by the Division to conduct the required oil&
grease analyses(Commercial Laboratory#248)as well the temperature and pH analyses
(Field Laboratory#5134). TRC and COD analyses are not required because no chemicals
are added in the electricity generating process. The records reviewed during the inspection
were organized and well maintained. Effluent, upstream, and downstream data were
reviewed for 2014-2016. No effluent limit violations and/or instream temperature
exceedances were reported. Facility conduct quarterly sampling:the permit requires
semiannual permit.

The Division has renewed the subject General Permit. We appreciate your patience during the
longer-than-expected renewal period. The Division hereby issues the updated version of Certificate of
Coverage (CoC) NCG500139 under General Permit NCG500000. It is issued pursuant to the requirements
of North Carolina General Statue 143-215.1 and the Memorandum of Agreement between North Carolina
and the US Environmental Protection agency dated October 15, 2007[or as subsequently amended].
If any parts, measurement frequencies or sampling requirements contained in this General Permit
are unacceptable to you,you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, the certificate of coverage shall be final and binding.
Please take notice that this Certificate of Coverage is not transferable except after notice to the
Division. The Division may require modification or revocation and reissuance of the certificate of coverage.
Contact the Mooresville Regional Office prior to any sale or transfer of the permitted facility.
Regional Office staff will assist you in documenting the transfer of this CoC.
This permit does not affect the legal requirements to obtain any other State, Federal, or Local

Good afternoon, Mr. Bell.This letter serves as Duke Energy's required written notification of the spill event at Cowan's
Ford Hydroelectric Station that was reported by telephone on Monday,August 31, 2015 at 10:26 am to the NRC and to
the NCDENR Mooresville office at 11:15 am.Thank you for your assistance in logging the details of this submittal.
Incident Description:While conducting routine daily inspection rounds at Cowan's Ford Hydro Station on 8/31/15,at
approximately 10:00am, a station tech discovered that oil had been released from Units 3/4 GSU transformer. The tech
immediately notified the station Interim Supervisor(IS), who notified the station Environmental Professional (EP) at
10:06. The tech had discovered that most of the oil had been released into the containment area, however, some oil
was released outside the containment onto the powerhouse concrete roof and appeared to have reached a roof storm
drain. The station EP requested that the station IS assess the tailrace area to see if any oil sheen was observed. Upon
assessing the tailrace area,the station IS notified the station EP at 10:18 that an oil sheen with a rainbow color was
observed on the tailrace waters. The station EP requested that the station IS inform Hydro Central of the incident and
request that all units remain shut down until further notice. Additionally,the EP requested that the station IS contact
the Upper Catawba Civil& Regulatory Compliance Supervisor to initiate deploying containment booms and absorbent
material in the tailrace via a boat. Both of these actions were immediately taken.
The station EP notified the National Response Center(NRC)of a non-PCB mineral oil release at 10:26 am. The quantity
of oil released into the containment area and the cause of the release was unknown at this time, however, it was
estimated that 1 pint or less was release into the tailrace waters via a roof storm drain. It was also unknown at this
point how any oil was released outside of the containment area. The EP notified the Duke internal spill hotline at 10:35
am. The spill duty person contacted the NCDENR at 11:11 am and was informed that the NCDENR had already received
electronic notification of the incident from the NRC. The duty person also notified the NCDENR—Mooresville Regional
Office at 11:15. The station EP notified the station manager of the incident at 10:53 am and the Environmental Field
Support Manager at 11:02 am. EPA region IV contacted the station EP at 11:06 am to verify details of the incident.
As mentioned,absorbent boom and pads were immediately deployed in the tailrace area. Additionally,station
employees immediately began cleanup of oil from the concrete roof surface,outside of the containment area. The
station EP, who was at Tuxedo Hydro Station when notified of the incident,arrived at Cowan's Ford Hydro at 11:50
am. Upon evaluating the situation, it is estimated that approximately 30 gallons or so of non-PCB mineral oil was
released into the transformer containment area. Apparently,the transformer oil cooling fans failed due to a faulty
voltage contactor,which caused the oil to overheat,expand,and overflow from the vent pipes on two oil conservatory
tanks,which are approximately 20'above ground level. It is also believed that the oil was released at sufficient force to
spray onto equipment below and somehow allow some of the oil to make it outside the containment area by spraying
and/or flowing across the conservatory tanks,which extend about 1 foot beyond the containment area below. At this
time, it is undetermined why cooling fan failure and/or high oil temperature alarms were not activated. It is believed
that this equipment failure and oil release occurred sometime on Sunday(the previous day)when the plant was
unstaffed. If it had not been for a lite rain event during the early morning hours of 8/31/15,the small amount of oil that
had been released outside of the containment area may not have reached the roof storm drain.
The transformer was subsequently de-energized and A&D Environmental Services is in the process of recovering
released oil from within the containment area and cleaning oil contaminated surfaces, both inside and outside the
containment.

NPDES permit expiration date is incorrectly listed as July 31, 2013 on page 3. The actual permit expiration date is July

PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS
Volume of wastewater to be permitted: The renewal application lists up to fourteen different
outfalls, however, some of these outfalls only discharge on a seasonal basis and some only
discharge during emergency by-passing of the dam. Many of these discharges are released into the
tailrace of the dam, and are difficult to differentiate as separate outfalls due to the fact they all mix
in a common area beneath the dam. This facility uses an oil/water separator for sump discharges
001, 004, 007, 010, and 013 to remove oil that might leak from the generators or associated
equipment. See the list found in the renewal application for specific flow rates for each outfall.

PART IV- EVALUATION AND RECOMMENDATIONS
The applicant, Duke Power Company, has applied for the renewal of coverage under General
Permit No. NCG500000 for the discharge of non-contact cooling water at the Cowans Ford Hydro
Station. The wastewater consists of non-contact cooling water,condensate, and dam seepage.
The oil/water separator connected to the outfalls listed in Part II above is there as a precautionary
measure to prevent any accidental oil spills to surface waters.
Pending a final review by the NPDES Unit, it is recommended that a certificate of coverage be
reissued as requested.

b) Site map, if the discharge is not directly to a stream, the pathway to the receiving stream must be
clearly indicated. This includes tracing the pathway of a storm sewer to its discharge point.
c) If this application is being submitted by a consulting engineer(or engineering firm), include
documentation from the applicant showing that the engineer(Or firm)submitting the application has
been designated an authorized Representative of the applicant.
d) Final plans for the treatment system (if applicable). The plans must be signed and sealed by a North
Carolina registered Professional Engineer and stamped-"Final Design-Not released for construction".

For Hydroelectric Facilities
The need has arisen for clarification of Part A(2) of the General Permit 500000 regarding Hydroelectric
Facilities. The permit requires monitoring for Temperature at the Effluent,Upstream and Downstream.
Upstream location for the natural water temperature shall be interpreted as the influent to the penstock or
other appropriate location in the impounded waterbody. Effluent is the outflow from the powerhouse.
The temperature change between these two values is considered the heat added and judged for compliance
with Note 1 of the permit and 15A NCAC 2H.0211 (j).

on the main stem of the Catawba River. The River is used for primary and secondary
recreation as well as general "C" classification uses. There is one known water supply
intake is located down stream in Mountain Island Lake.
PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS
1. a. Volume of wastewater to be permitted: The renewal application lists up to fourteen
different outfalls, however, some of these outfalls only discharge on a seasonal basis and
some only discharge during emergency by-passing of the dam. See list in renewal
application for specific flow rates for each outfall.
b. Current permitted capacity of the wastewater treatment facility: The current permit does not
list a permitted capacity.
c. Actual treatment capacity of the current facility (current design capacity): There are no
existing treatment facilities.
d. Date(s) and construction activities allowed by previous Authorizations to Construct issued
in the previous two years: There have been no ATC's issued to this facility in the past two
years.
e. Please provide a description of existing or substantially constructed wastewater treatment
facilities: The facility uses an oil/water separator for sump discharges (001, 004, 007, 010,
013)to remove oil that might leak from the generators or associated equipment.
f. Please provide a description of proposed wastewater treatment facilities: There are no
proposed WWT facilities at this time.
g. Possible toxic impacts to surface waters: None that we are aware.

The applicant, Duke Power Company, has applied for the renewal of coverage under General
Permit No. NCG500000 for the discharge of non-contact cooling water at the Cowans Ford Hydro
Station. The wastewater is non-contact cooling water and condensate. The oil/water separator is used
as a precautionary measure to prevent any accidental oil spills to surface waters.
Pending a final review by the NPDES Unit, it is recommended that a certificate of coverage be
reissued as requested.

Dear Permittee:
The facility listed above is covered under NPDES General Permit NCG500000. NCG500000 expires
on July 31, 2007. Federal(40 CFR 122.41) and North Carolina(15A NCAC 2H.0105(e))regulations require
that permit renewal applications must be filed at least 180 days prior to expiration of the current permit. If
you have already mailed a renewal request,you may disregard this notice.
To satisfy this requirement,the Division must receive a renewal request postmarked no later than
February 1, 2007. Failure to request renewal by this date may result in a civil penalty assessment. Larger
penalties may be assessed depending upon the delinquency of the request. This renewal notice is being sent
well in advance of the due date so that you have adequate time to prepare your application.
If any discharge previously covered under NCG500000 will occur after July 31, 2007, the
CoC must be renewed. Discharge of wastewater without a valid permit would violate North Carolina
General Statute 143-215.1; unpermitted discharges of wastewater may be assessed civil penalties of up to
$25,000 per day.

December 6, 1983.

cooling water at the Cowans Ford Hydro Station .
The wastewater is non-contact cooling water and condensate .
The oil/water separator is used as a precautionary measure to
prevent any accidental oil spills to surface waters .
It is recommended that coverage under the General Permit be
granted and a certificate of coverage be issued .
