Incident Description:While conducting routine daily inspection rounds at Cowan's Ford Hydro Station on 8/31/15,at approximately 10:00am, a station tech discovered that oil had been released from Units 3/4 GSU transformer. The tech immediately notified the station Interim Supervisor(IS), who notified the station Environmental Professional (EP) at 10:06. The tech had discovered that most of the oil had been released into the containment area, however, some oil was released outside the containment onto the powerhouse concrete roof and appeared to have reached a roof storm drain. The station EP requested that the station IS assess the tailrace area to see if any oil sheen was observed. Upon assessing the tailrace area,the station IS notified the station EP at 10:18 that an oil sheen with a rainbow color was observed on the tailrace waters. The station EP requested that the station IS inform Hydro Central of the incident and request that all units remain shut down until further notice. Additionally,the EP requested that the station IS contact the Upper Catawba Civil& Regulatory Compliance Supervisor to initiate deploying containment booms and absorbent material in the tailrace via a boat. Both of these actions were immediately taken. The station EP notified the National Response Center(NRC)of a non-PCB mineral oil release at 10:26 am. The quantity of oil released into the containment area and the cause of the release was unknown at this time, however, it was estimated that 1 pint or less was release into the tailrace waters via a roof storm drain. It was also unknown at this point how any oil was released outside of the containment area. The EP notified the Duke internal spill hotline at 10:35 am. The spill duty person contacted the NCDENR at 11:11 am and was informed that the NCDENR had already received electronic notification of the incident from the NRC. The duty person also notified the NCDENR—Mooresville Regional Office at 11:15. The station EP notified the station manager of the incident at 10:53 am and the Environmental Field Support Manager at 11:02 am. EPA region IV contacted the station EP at 11:06 am to verify details of the incident. As mentioned,absorbent boom and pads were immediately deployed in the tailrace area. Additionally,station employees immediately began cleanup of oil from the concrete roof surface,outside of the containment area. The station EP, who was at Tuxedo Hydro Station when notified of the incident,arrived at Cowan's Ford Hydro at 11:50 am. Upon evaluating the situation, it is estimated that approximately 30 gallons or so of non-PCB mineral oil was released into the transformer containment area. Apparently,the transformer oil cooling fans failed due to a faulty voltage contactor,which caused the oil to overheat,expand,and overflow from the vent pipes on two oil conservatory tanks,which are approximately 20'above ground level. It is also believed that the oil was released at sufficient force to spray onto equipment below and somehow allow some of the oil to make it outside the containment area by spraying and/or flowing across the conservatory tanks,which extend about 1 foot beyond the containment area below. At this time, it is undetermined why cooling fan failure and/or high oil temperature alarms were not activated. It is believed that this equipment failure and oil release occurred sometime on Sunday(the previous day)when the plant was unstaffed. If it had not been for a lite rain event during the early morning hours of 8/31/15,the small amount of oil that had been released outside of the containment area may not have reached the roof storm drain. The transformer was subsequently de-energized and A&D Environmental Services is in the process of recovering released oil from within the containment area and cleaning oil contaminated surfaces, both inside and outside the containment. 1 If you have any questions about the details of this incident,do not hesitate to contact me. Upstream location for the natural water temperature shall be interpreted as the influent to the penstock or other appropriate location in the impounded waterbody. Volume of wastewater to be permitted: The renewal application lists up to fourteen different outfalls, however, some of these outfalls only discharge on a seasonal basis and some only discharge during emergency by-passing of the dam.
