The North Carolina Division of Water Resources conducted an inspection of the Slash Creek WWTP on
11/19/2020. This inspection was conducted to verify that the facility is operating in compliance with the
conditions and limitations specified in Non -discharge Permit No. WQ0022725. The findings and
comments noted during this inspection are provided in the enclosed copy of the inspection report
entitled "Compliance Inspection Report".
There were no significant issues or findings noted during the inspection and therefore, a response to this
inspection report is not required.

Inspection Summary:
On 11/19/2020 from 11:00am to 12:30pm, Paul Mays and Robert Tankard with the Division of Water Resources from the
Washington Regional Office conducted an inspection of Slash Creek WWTP operating under permit#WQ0022725. The
facility was found to be non -compliant with permit #WQ0022725. Below are the findings during the inspection:
The backwash function via air scouring for the tertiary filter was not operational during the inspection. The backup generator
was not operational and the "warning light" was on for the turbidity meter during the inspection. When the generator was
further inspected the display on the generator pointed to an ignition fault. ORC David Phar stated the battery died the
morning of the inspection. Michelle Pharr followed up with photo evidence that the generator, the turbidity meter, and
backwash function of the tertiary filter are now fully operational. These photos are included in the report and docemented.
The Turbidity meter was calibrated on 05/16/2020
Effluent flowmeter was calibrated on 05/01/2020
The 10' x 12' x 13' aeration basin appeared to be fully operation. However, the entire basin was covered with a thick foam. It
was unknown what caused the foam layer to form on the basin.
All records and reports requested for the inspection were available. Operation and maintenance records were available for
review. Upon review of the 07/2020 NDMR discrepancies for total phosphorous, PH, TSS and DO were found between the
report and the lab data. On 07/15/2020 PH was 7.46 for the NDMR and 7.46 from lab data, total phosphorous was 0.73mg/I
and not found in lab data, TSS was 3.4mg/I in NDMR and 4.55mg/I from lab data and lastly, DO was 3.4mg/I in NDMR and
4.55mg/I in lab data. The 06/2020 NDMR was spot checked with no discrepancies. Michelle Pharr will be followed up with an
amended 07/2020 NDMR and corresponding lab data. No discrepancies were found within the amended report.
At the time of inspection many of the irrigation nozzles were not fully functional and did not irrigate properly. Due to this the
specified acreage in the permit was not being irrigated. The irrigation nozzles require maintenance or fixing in order to
correct this issue. Documentation has been provided showing that the irrigation system was fully functional after the
inspection by Michelle Pharr. This documentation is included at the end of the report and documented.

Comment: All records and reports requested for the inspection were available. Operation and maintenance

Dear Mr. Mays,
Per your request, as faltow up to the November 19, 2020 Slash Creek impaction, I am providing you with written documentation of the noidng status of the Slash Creek Irrigation System. There are 16 spray irrigation acmes, 1,2,5.6,7,8,9,10,11,12,13,14,15,16,17,18, with B to 10
spray IrrlgaUon nozzles per zone. Post Inspection, all zones ware chocked and ab nozzles are working.
Than have boon 5 spray nozzles Identified, when activated, are not consistent with their spray pattern. The owners have now winterized the Irrigation system. We will work on the nozzles that have Irregular spray performance In the spring when the krigatfon system Is Initialized.
