Division of Water Resources INSTRUCTIONS FOR FORM CSA 04-16&SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General—When submitting an application to the Pretreatment,Emergency Response,&Collection Systems(PERCS)Unit,
please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these
instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of requested
additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
A. Cover Letter RECEIVED
▪ Submit a cover letter listing all items and attachments included in the permit application package DEC 14 2020

C. System-Wide Wastewater Collection System (FORM: CSA 04-16)Application:
❑ Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
❑ If the Applicant Type in Section 1.3 is a Privately-Owned Public Utility, provide the Certificate of Public
Convenience and Necessity(CPCN)from the North Carolina Utilities Commission demonstrating the Applicant is
authorized to hold the utility franchise for the area to be served by the wastewater collection system,or
❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a franchise has been received and that the service area is contiguous to an existing franchised area
or that franchise approval is expected.
❑ if the Applicant Type in Section 1.3 is a corporation or company, provide documentation if it is registered for
business with the North Carolina Secretary of State.

• The Authorized signing official listed in Section 1.4 should match with that of the Applicant certification page in
accordance with 15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated
as the signing official if a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T
.0106(b).

why we have meters at each account's address set in the
city's right-of-way (ROW) with over 6,000-metered
connections. The meters that we use are electronic self-
reading meters. Each meter is equipped with a meter
transmission unit (MTU) that sends the reading to a data
collection unit (DCU)that runs solely on solar energy.The
DCU then sends all of the meter readings to a server located
at City Hall. If your meter is indicating the potential of a leak
at your residence or business, an employee generates a
notification to inform you of our findings so that any repairs
needed may be facilitated before anything gets out of

Lift Stations & Force Mains
Ensuring the safe and sanitary transportation of wastewater from sewer customers to the WWTP.

The Town of Morehead City tries to improve and update at
least one Lift Station a year.Taking a proactive stance on
maintaining and updating the existing facilities allows the
city to spread the high cost of retrofitting these assets over i
several years. This saves the city money and lessens the
likelihood of having to replace several at one time. Given
that the cost of upgrading just one Lift Station can be
approximately half a million dollars, it could be a huge
financial burden to the city if several had to upgraded at onelripp
time.The city has 37 Lift Stations in total, and to upgrade If
them all at once would cost the city roughly ten million
dollars. This means proper maintenance from skilled
employees is essential to the longevity of these very
important apparatuses.
These lift stations help to move wastewater to the WWTP;
therefore, it could be a hazardous situation if even one
were to malfunction. This is why they are all checked for
proper operation, cleaned, and maintained daily by a •
WWTP employee.Each lift station in town is also monitored
by a system called SCADA (Supervisory Control and Data
Acquisition).SCADA alerts the city when there is a problem
and then a technician is sent to remedy the issue.

backing up and causing health concerns. Even during
catastrophic events like Hurricane Florence in 2018 where
there was a prolonged period of no electricity, WWTP
employees were able to keep the lift stations operating so
that the city never loss sewer service.

PURPOSE
The purpose of this memo is toprotect the citizens of the Town of Morehead Cityand the environment

by establishing written procedures for responding to Sanitary Sewer Overflows. This procedure applies
to all water and sewer department employees,which in the course of their work, may deal with SSO's.
SSO's endanger our environment and potentially the health of any citizen or employees that come in
contact with an SSO. Effective immediately,the"point"system will no longer be used. Any SSO is
subject to a possible enforcement action such as a Notice of Violation and/or a civil penalty assessment.
1. Upon discovery of an SSO,an initial assessment will be made by the personnel responding
to a complaint. If an event has taken place,or is in progress,the employee will eliminate
the source of the SSO with one or more of the following methods:
a) Call for backup if a blockage appears to be the problem and mobilize the jet rodder
to correct the problem.
b) Contact the WWTP for personnel if the Lift Station does not appear to be
functioning properly.
c) Set up a pump to bypass the wastewater around the blockage,from manhole to
manhole.
d) Contact wastewater haulers to haul wastewater.
e) If the problem appears to be the customer's problem, notify the customer as soon
as possible to take corrective action.
2. Clean up and containment procedures include the following:
a) Chlorinate wastewater from SSO's that enters ditches or surface waters.
b) Place lime on areas of contamination in the customer's yard or in grassy areas.
c) Wash off asphalt or concrete areas with the jet rodder.
d) Remove all visible debris and ponded wastewater.
e) If the soil is disturbed,the area should be reseeded to prevent erosion.
f) Place barriers and/or tapes around affected area for 24 hours in non-traffic areas.
3. The WWTP or Line Maintenance Supervisors shall fill out the Collection System Sanitary
Sewer Overflow Form,and the Sewage Spill Response Evaluation for all spills,and report as
follows:
a) Use the Collections System Sanitary Overflow Form for SSO that occurs in the
collection system and at lift stations.
b) Use the Sewage Spill Response Evaluation,for SSO that occur at the wastewater
treatment plant.
c) A reportable SSO is a greater than 1,000 gallons to the ground or an SSO is any
amount that reaches surface water(including through ditches, storm drain, etc...)
d) Report by telephone,to a person at the State Regional Office within 24 hours if any
wastewater enters the surface waters or spills greater than 1,000 gallons on the
ground, regardless whether they were contained.
e) The time frame for submittal of both Part 1 and Part 2 for the Collection System
Sanitary Sewer Overflow Form is five(5)working days.

Per G.S. 143-215.1C(b),the owner or operator of any wastewater collection system shall:
In the event of a discharge of 1,000 gallons or more of untreated wastewater to the surface waters of the State, issue
a press release to all print and electronic news media that provide general coverage in the county where the
discharge occurred setting out the details of the discharge. The press release shall be issued within 24 hours after
the owner or operator has determined that the discharge has reached surface waters of the State.
In the event of a discharge of 15,000 gallons or more of untreated wastewater to the surface waters of the State,
publish a notice of the discharge in a newspaper having general circulation in the county in which the discharge
occurs and in each county downstream from the point of discharge that is significantly affected by the discharge. The
Regional Office shall determine which counties are significantly affected by the discharge and shall approve the form
and content of the notice and the newspapers in which the notice is published.

Regarding: Pump Station Emergency Response

PURPOSE
The purpose of this memo is to protect the citizens of the Town of Morehead City and the environment by
establishing written procedures for responding to Pump Station Alarm Conditions. This procedure applies
to all water and sewer department employees,which in the course of their work,may deal with Pump
Station Alarms.
Pump Station Alarms alert us to emergency conditions that could result in a Sewer System Overflow.
Timely response may eliminate potential environmental damage as well as dangerous health effects for
our citizens and employees.
Upon notification of an alarm condition,the personnel responding to the alarm shall inspect the lift station
and evaluate the cause of the alarm condition. If a pump station failure is occurring,the employee shall
resolve the condition by one or more of the following methods:
a) Call for backup if a blockage appears to be the problem and mobilize the jet rodder to correct the
problem.
b) Contact the WWTP for personnel if the Lift Station does not appear to be functioning properly.
c) Set up a pump to bypass the wastewater around the blockage, from manhole to manhole.
d) Contact wastewater haulers to haul wastewater.
e) If the problem appears to be the customer's problem, notify the customer as soon as possible to
take corrective action.
If the emergency condition results in a sewer system overflow or spill, follow the Sanitary Sewer
Overflow Response Plan.
