RE: Notice of Violation NOV-2020-DV-0455
1. After reading your violation notice several times, I am happy to clarify that my organization
believed that we had filed an SSO Report on September 9111, which I have forwarded to Mr.
Mitchell Hayes on November 12"', 2020 via email. I am including a copy of the email that my
Vice -President had submitted along with the SSO.
2. The timeline of events and numerous details for the repair are submitted in the SSO which I
will not repeat, but I would assert that we responded to this line breakage in an expeditious
manner to protect our valuable surface water resources. Please consider our efforts and expenses
so far as mitigating factors if you still plan on imposing a civil penalty.
3. Please note that in the root -cause analysis of the line breakage, it was recommended that we
insulate this section of pipe that is totally exposed to the elements. We are proceeding with this
recommendation also to insulate the approximately 100' of pipe along the bridge and waterway.
I have included one estimate we have received for this improvements, but we are also
considering other options, but we are committed to insulating that pipe before the weather gets
too cold.
4. This wasteline is not heavily used, but it is an important consideration for our Gatehouse
personnel. If there are other actions your office would recommend we take in lieu of fines or
revocation of this use permit, please let me know.

Thank you for entrusting Pipeworx Plumbing with your plumbing needs. Our mission is
to provide you with professional and quality workmanship in accordance with North
Carolina State Plumbing Code that is also completed in a safe and timely manner.
• Replace 100' of 2" pressure pipe with insulated pipe on the bridge
• Materials will include (but not limited to) 100' 2" pressure pipe, 100' 2" PVC
insulation with UV resistant insulation protection cover on all exposed
piping.
• Carolina Trace must provide a boat with a driver
• Carolina Trace must provide bridge closure or traffic controller as at least
one lane must be shut down
TOTAL: $ 8400.00
This estimate includes all parts and labor for the above work only.
This estimate does not include additional plumbing beyond what is listed above.
This estimate does not include any unforeseen issues that may arise.
All material is guaranteed to be as specified for one year. All work to be completed in a workmanlike manner
according to standard practices. Any alteration or deviation from above specifications involving extra costs will be
executed only upon written orders and will become an extra charge over and above the estimate. Any agreements
contingent upon strikes, accidents, or delays beyond our control. Owner to carry fire and other necessary
insurance. Our workers area fully covered by Workman's Compensation Insurance.
