No.2 Fuel Oil Spill Incident
Occurring on January 5,2004
Gaston County
Dear Mr.Leslie:
A spill of no. 2 fuel was discovered at approximately 9:00 am on Monday, January 5, 2004 by
Scottie Theel; the boiler operator. The location was Stowe Downtown Complex; Continental
Boiler fuel oil feed return line where the piping enters the ground to the 7,500 gallon
underground storage tank Facility ID #: 0-029136, Certificate #: 2003083460. A leak of an
estimated 50 gallons was discharged on the ground with the remaining oil traveling west 10 feet
to the parking lot pavement and then 50 feet to a storm water drain discharging into an unnamed
tributary where it was contained approximately 100 feet downstream. The petroleum
contaminated soil was removed and transported to Environmental Soils, Inc. Non-Hazardous
Waste Mainfest# 16145 & 16146. The approximately 10 to 20 gallons of contained fuel oil on
the water was removed by skimming and oil absorbent materials. HAZ-MAT Transportation
and Disposal. Inc was the contractor performing the clean up and disposal operation. Final
disposal of the water and oil was their facility for fuel oils blending, Non-Hazardous Special
Waste Manifest # 19391 and 19499. Please find attached copies of the above manifest. There
was no fish kill or any evidence of contamination of the stream.
When the spill was discovered our Pharr Yarns Emergency Pollution Incident Procedures were
enacted as follows:
0900: Spill was discovered by Scottie Theel.
0900:The fuel oil pump was turned of and locked out which stopped the leak.
0905: Mike Church,Environmental Engineer,and Jimmy Simpson,plant engineer was
contacted.
0910: Our Emergency Response Contractor,HAZ-MAT was contacted.Jack Holder took our
information and advised that they would leave with the required equipment and personnel
within 15 minutes.Pharr Yarns personnel responded,placed oil absorbent booms in the
stream to contain the fuel oil,and also at the out fall of the storm drain into the stream.
1000: HAZ-MAT arrived and began the clean up.
1700: HAZ-MAT had removed most of the oil and had contained the remaining oil sheen with
absorbent booms and pads.The decision was made to continue the operation the next
morning at daylight.

0830:Tuesday 1/6/04.HAZ-MAT returned and continued the clean up.
0900:The contaminated soil was removed and transported to Environmental Soils.
The leak was corrected by replacing the piping at the point of the leak.The
remaining piping was inspected and found to be in excellent condition.
1600: HAZ-MAT concluded the clean up
1630: Mike Church contacted the NC DENR, Water Quality Section,Mr.John
Leslie to report the spill and advise of the completion with no harm to the
Environment.

Dear Mr. Bou-ghasale:
Please find attached our report of the fuel oil spill that was reported by telephone on
April 21 s`. The spill was contained and the contaminated soil removed for disposal
by our spill response contractor. We estimated that approximately 85 gallons was
captured near the facility and approximately 15 gallons was removed in the
containment area described in the report.

PHARR
YARNS 4:45 PM I was advised by James Davis of a spill of No. 6 fuel oil
from the Space Dye boiler room traveling east and downhill
toward the South Fork River where it was contained by natural
ground topography. However heavy rain caused some of the fuel
oil to enter a natural drainage ditch. The river had risen to three
feet above flood level, causing concern about the fuel oil entering
the river.
4:50 PM I called our Emergency Response Contractor, Haz-Mat, Jack
Holder, who advised that all his equipment was in use and that
he would contact Hepaco for immediate response.
5:00 PM Bob Baxter of Hepaco called and I advised him of our needs.
James Davis called Vernan Sherrill Backhoe Service who arrived
within minutes and removed most of the oil contaminated soil
• and placed it on plastic for disposal. George Altice placed
two floating booms from our emergency inventory below the
spill in the old abandoned raceway prior to reaching the river.
7:00 PM Hepaco arrived, placed an 18 inch floating boom on the
flooded area below the two existing booms and the river to
make sure that no fuel could reach the river, and began their
cleanup procedures.
Saturday April 19, 2003
12:00 AM The cleanup continued at the contained area by Hepaco and their
vac truck was used at the boiler room to clean up the fuel oil spill.
02:00 AM Any remaining fuel oil was contained and the river level was
receding, the clean up was discontinued until the next morning.

7:00 AM Hepaco arrived and began removing the remaining oil with a
floating skimmer and absorbent material.
3 :00 PM Hepaco completed their cleanup service.

9:00 AM After the river receded to normal level, some of the remaining
fuel oil on the banks of the contained area had attached
to vines, other vegetation, and debris. Absorbent material was
placed in the old raceway area upstream of the floating booms.
Straw was applied to the banks and a controlled burn with
post cleanup accomplished the final removal of fuel oil.
April 28, 2003
9:00 AM Final removal of the contaminated soil was transported by
Haz-Mat for approved disposal.
Cause of the spill: A leak of approximately 100 gallons in the boiler
recirculation piping system located near the pump was not detected. The
drainage ditch from the boiler room to the oil separation / pumping station
overflowed outside of the boiler room at an open 90 degree connection box to
underground piping ending at the oil separation / pumping station. The spill
occurred when the water softener automatically backwashed causing the ditch
to overflow. Failure of the oil alarm and wastewater transfer pumping station
allowed the drain to overflow.
Corrective action taken to prevent future spills New and improved pump
controls have been installed which alternate the dual pumps and provide a back
up in case of pump failure. A purchase order has been issued for a state of the
art fuel leak detection alarm. The controller output will be connected to our
plant security 24 hour call service for the oil detection alarm and high water
alarm for the pumping system. Inspection of all of the piping from the fuel oil
underground storage tanks to the boiler room was conducted and all piping
with evidence of corrosion has been replaced.
Closer attention to the existing Pharr Yarns Environmental Procedures which
include the Fuel Oil SPCC Check List and Storm Water Pollution Prevention
Plan Check List along with the physical improvements will help to prevent and
insure that no future oil spills occur.
