Dear Permittee:
In accordance with your application for permit renewal,the Division is forwarding the subject NPDES general
permit for petroleum contaminated groundwater remediation systems. This General Permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated December 6, 1983 and as subsequently amended.
The Certificate of Coverage previously issued to your facility is now valid under the revised General Permit for
discharges associated with groundwater remediation systems(NCG510000-issued August 22,1996). The new General
Permit is more comprehensive, in that it covers remediation of groundwater contaminated with heavier fuels (such as
diesel, fuel oil, and kerosene) as well as gasoline contamination. Please note that the permit has three effluent
limitations pages. The type of contamination at your site (gasoline only, heavy fuels only, or gasoline and heavy
fuels) will determine the effluent limitations for your discharge.
If any parts,measurement frequencies or sampling requirements contained in this permit are unacceptable to you,you
have the right to request an individual permit by submitting an individual permit application. Please take notice
that the previously issued Certificate of Coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the Certificate of
Coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division of
Water Quality or permits required by the Division of Land Resources,the Division of Coastal Management, or any
other Federal or Local governmental permit that may be required.

Dear Mr. Overton:
In accordance with your application for an NPDES discharge permit received February 21, 1996
by the Division, we are herewith forwarding the subject Certificate of Coverage under the state-NPDES
general permit for the Springwood Facility and one set of final approved plans and specifications.
Authorization is hereby granted for the construction and operation of a 100 GPM groundwater recovery
and treatment system consisting of an oil/water separator, free product storage tank, tray-type air stripper,
particulate filter and granular activated carbon units with final discharge of treated groundwater to an
unnamed tributary of the South Fork Catawba River. This Certificate of Coverage is issued pursuant to
the requirements of North Carolina and the US Environmental Protection Agency Memorandum of
Agreement dated December 6, 1983 and as subsequently amended.
Please note that this Certificate of Coverage requires that all three major treatment components (the
oil/water separator, the air stripper and the carbon units) remain in operation throughout the life of this
facility.
If any parts, measurement frequencies or sampling requirements contained in this general permit
are unacceptable to you, you have the right to submit an individual permit application and letter requesting
coverage under an individual permit. Unless such demand is made, this decision shall be final and
binding. Please take notice this permit is not transferable. Part II, E.4. addresses the requirements to be
followed in case of change of ownership or control of this discharge.
The Authorization to Construct is issued in accordance with Part III, Paragraph 2 of NPDES
Permit No. NCG510000, and shall be subject to revocation unless the wastewater treatment facilities are
constructed in accordance with the conditions and limitations specified in Permit No. NCG510000.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required by this
Division, such as the construction of additional or replacement wastewater treatment or disposal facilities.
The Mooresville Regional Office,telephone number(704) 663-1699 shall be notified at least forty-
eight (48) hours in advance of operation of the installed facilities so that an in-place inspection can be
made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00
a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.

Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been installed in
accordance with the NPDES Permit, the Certificate of Coverage, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box
29535, Raleigh, NC 27626-0535.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for the
life of the facility.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina
General Statute 143-215.6A to 143-215.6C.
The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be required by the Division of Water Quality or
permits required by the Division of Land Resources,the Coastal Area Management Act or any Federal or
Local other governmental permit that may be required.

2000) and travel approximately 1 .3 mile to the intersection
with Park Terrace Drive (SR 2104) . Gilliland's Place is
located on Hickory Grove Road just south of the Park Terrace
Drive intersection.

and found not to be an alternative.
Connection to regional Sewer System: Not an economically
feasible alternative according to applicant. The nearest

sanitary sewer system is located approximately one mile from
the site.
Discharge to an infiltration gallery: The use of an
infiltration gallery was evaluated and found not to be a
viable alternative given the presence of bedrock at a shallow
depth.
5 . Air Quality and/or Groundwater concerns or hazardous materials
utilized at this facility that may impact water quality, air
quality or groundwater? Contaminated groundwater already
exists at the site. No hazardous waste will be generated.
Air Quality Permit for this facility is not required.
However, registration with the Air Quality Section is
required.
PART IV - EVALUATION AND RECOMMENDATIONS
Pacemaker Leasing Company is requesting a General NPDES Permit
for the discharge of treated groundwater resulting from a leaking
underground storage tank.
The discharge pipe crosses adjacent property; therefore, an
easement agreement from all involved parties should be provided
prior to the issuance of the permit.
The effluent from similar facilities has been known to fail
toxicity testing.
It is recommended that the NPDES Permit be issued once the
above mentioned concerns regarding easement agreement and toxicity
have been addressed.

Specifications for the above-referenced groundwater treatment facility were
submitted to you as part of an application for a general NPDES permit on February 13,
1996. An addendum containing information regarding a tray aeration system was
submitted on April 26, 1996. Enclosed is an updated figure which shows the proposed
location of the discharge line from the groundwater treatment system to the point of
discharge into the unnamed tributary of the South Fork of the Catawba River. This
figure replaces Figure 3 from the NPDES permit application dated February 13, 1996.
