Dear Permittee:
NPDES General Permit NCG510000, under which the subject facility is covered, expires on
September 30, 2006. North Carolina Administrative Code (15A NCAC 2H.0105(e))requires that an
application for permit renewal be filed at least 180 days prior to expiration of the current permit. To satisfy
this requirement, your renewal package should have been sent to the Division postmarked no later than
April 3, 2006.
The Division has not received a renewal request for the subject CoC. This is a violation of your
permit at Part II. B. 10, which states"Any permittee that has not requested renewal at least 180 days prior
to expiration...will subject the permittee to enforcement procedures as provided in NCGS 143-215.6 and 33
USC 1251 et. seq.".
To prevent an assessment of civil penalties you must submit a completed permit application(see
enclosed forms) no later than June 19, 2006. If the system is inactive due to lack of trust fund payments,
but the remediation equipment is still on site, the CoC should be renewed.

Dear Mr. Overton:
In accordance with your application for an NPDES discharge permit received February 21, 1996
by the Division, we are herewith forwarding the subject Certificate of Coverage under the state-NPDES
general permit for the Springwood Facility and one set of final approved plans and specifications.
Authorization is hereby granted for the construction and operation of a 100 GPM groundwater recovery
and treatment system consisting of an oil/water separator, free product storage tank, tray-type air stripper,
particulate filter and granular activated carbon units with final discharge of treated groundwater to an
unnamed tributary of the South Fork Catawba River. This Certificate of Coverage is issued pursuant to
the requirements of North Carolina and the US Environmental Protection Agency Memorandum of
Agreement dated December 6, 1983 and as subsequently amended.
Please not:that this Certificate of Coverage requires that all three major treatment components (the
oil/water separator, the air stripper and the carbon units) remain in operation throughout the life of this
facility.
If any parts, measurement frequencies or sampling requirements contained in this general permit
are unacceptable to you, you have the right to submit an individual permit application and letter requesting
co-ycragy cndc_ ar in i•'iaua ,.ennit. unit-Fs s'irl, demand is made, this decision shall be final and
binding. Please take notice this permit is not transferable. Part II, E.4. addresses the requirements to be
followed in case of change of ownership or control of this discharge. .
The Authorization t-) Construct is issued in accordance with Part III, Paragraph 2 of NPDES
Permit No. NCG510000, an-1 shall be subject to revocation unless the wastewater treatment facilities are .
constructed in accordance with the conditions and limitations specified in Permit No. NCG510000.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required by this
Division, such as the construction of additional or replacement wastewater treatment or disposal facilities.
The Mooresville Regional Office,telephone number(704) 663-1699 shall be notified at least forty-
eight (48) hours in advance of operation of the installed facilities so that an in-place inspection can be
made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00
a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.

Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been installed in
accordance with the NPDES Permit, the Certificate of Coverage, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box
29535, Raleigh, NC 27626-0535.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for the
life of the facility.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina
General Statute 143-215.6A to 143-215.6C.
The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, the Coastal Area Management Act or any Federal or
Local other governmental permit that may be required.

February 13, 1996. Enclosed is an addendum containing information regarding a tray
aeration system which has been selected in lieu of a packed tower air stripping system.
The information provided in the addendum is based on one manufacturer; however,
equivalent models from other manufacturers will be considered acceptable.

Dear Mr. Overton:
The Division acknowledges receipt of your NPDES permit application for coverage under general
permit number 510000 and a$550 check (#70522) received February 21, 1996.
After a preliminary review of the proposed treatment system, several questions have surfaced. Our
primary concern is the ability of the system to meet effluent limits set forth in the general permit. The
general permit for groundwater remediation systems has an effluent limit for toluene of 11 141. Evidence
that this limit will be met must be provided before a Certificate of Coverage can be issued for this facility.
Furthermore, the general permit will expire July 31, 1997. When the permit is renewed it will
include provisions for discharges into water supply waters. The permit limits for these discharges will
likely be much more stringent than for the current general permit. This fact should be considered in
designing the said treatment system.
Final plans sealed by a North Carolina professional engineer must be submitted to the Division
before an Authorization to Construct can be issued for this facility. A decision must be made on the proper
type of air stripping to be used. Once this decision is made, plans specifying this decision must be
submitted to the Division.
I am, by copy of this letter, requesting that the Mooresville Regional Office Supervisor prepare a
staff report and recommendations regarding this discharge. This application has been assigned to me for
