401/Wetlands Unit, North Carolina Division of Water Quality, 1650 Mail Service Center, Raleigh, NC,
27699-1650. This form may be returned to DWQ by the applicant, the applicant's authorized agent, or
the project engineer. It is not necessary to send certificates from all of these.

The Division of Water Quality received the Certificate of Completion for DWQ #03-0639, Arbor Commons Subdivision
(Wake County) on August 30, 2010. The database has been updated accordingly to reflect receipt of the certificate. The
401 Certification was issued July 25, 2005. The receipt of the Certificate of Completion indicates that all project work
approved within the 401 Water Quality Certification or applicable Buffer Rules, and any subsequent modifications has
been carried out to completion.
