All,
On December 3, 2020 Myself, Alaina Morman, Aana Gamble, Tim Heim and myself visited the AZP facility for a follow-up
inspection. We met with Charles Howell and Kobus DeWet on site.
Below are my major notes from our inspection on December 3, 2020 of the AZP facility.
1) Weekly monitoring was conducted, but not at the designated location. After speaking with Kobus DeWet on site,
they will now be sampling at the correct location in the coming months. I have requested that Charles Howell
send me all monitoring data for the past 3 months, and will be sending him an email to request as well.
2) While Tim Basilone wrote that the facility was not in Tier 3 due to when operations began, they have now
moved to Tier 3 just with their monitoring done in 2020. They have acknowledged this and sent DEMLR a
notification of Tier 3 status.
3) Secondary containment issues have all been addressed. I discussed the need for inspection frequency of
secondary containment areas with Mr. Howell while on site.
4) pH readings were taken in the riser of the SW basin (6.83 pH (14.5 degrees Q. At the inlet to the basin (6.96 pH
(17 degrees Q. In a manhole adjacent to spill location (5.86 pH (16.4 degrees Q.
5) The 5.86 pH was recorded above the point at which the NCG50 discharge comes into the SW system, and a non-
stormwater flow was present.
6) AZP has begun installing an automatic sampler at the inlet to the SW basin.
7) We discussed the possible paths to addressing the monitoring issues. AZP staff are unsure at this time what is
causing the metals issues.
Due to their understanding of where the correct sampling is to be done, their installation of a sampler, the pH
measurements taken on site, and the location sampling is needed: I feel that this may not be a good application for the
continuous pH monitoring mentioned in my previous email.
Mr. Howell wants to discuss missing corrective actions from the initial NOV issued after the fire. Please let me know if
you would like to be in on that call.
Over the next few weeks, I would like to have a follow-up discussion once Tim Heim has his sample results, and once Mr.
Howell gets his results to me.
