Swine Waste Collection,Treatment, Storage and Application System _ Sampson County Dear David J Mixon: • - The groundwater monitoring requirement for your facility was inadvertently left out from your previously issued Certificate of Coverage(COC), and we are hereby forwarding to you this corrected COC issued to David J Mixon, authorizing the operation of the subject animal waste management system in accordance with General Permit AWG100000.Please read this COC and the enclosed State General Permit carefully. This approval shall consist of the operation of this system including, but not limited to, the management and land application of animal waste as specified in the facility's Certified Animal Waste Management Plan (CAWMP)for Mixon Family Farms,Inc.,located in Sampson County,'with a swine animal capacity of no greater than the following annual averages: Wean to Finish: Feeder to Finish: 6978 Boar/Stud: Wean to Feeder: Farrow to Wean: Gilts: • Farrow to Finish: Farrow to Feeder: Other: Please pay particular attention to the attached groundwater monitoring requirements that are herebyincorporated into the permit'for this facility. If this is a Farrow to Wean or Farrow to Feeder operation,there may be one boar for each 15 sows.Where boars are unnecessary, they may be replaced by an equivalent number of sows. Any of the sows may be replaced by gilts at a rate of 4 gilts for every 3 sows. _ __ This COC shall be effective from the date of issuance until September 30, 2024-and shall hereby void - - Certificate of Coverage Number AWS820020 that was,previously issued to this facility.-Pursuant to this COC, you are authorized and required to operate the.system in conformity with the conditions and limitations as specified in the General Permit,the facility's CAWMP, and this COC. An adequate system for collecting and maintaining the required monitoring data and operational information must be established for this facility. Any increase in waste production greater than the certified design capacity or increase in number of animals authorized by this COC(as provided above)will require a modification to the CAWMP and this COC and must be completed prior to actual increase in either wastewater flow or number of animals. You are encouraged to update your Swine Odor Control Checklist using the enclosed form. If you do so, you must send a copy of the updated form to the Animal Feeding Operations Program at the address below. Stocking and Mortality Form (STOCK-1) has been updated; all other record keeping forms are unchanged with this General Permit. Please use the most current record keeping forms. If your Waste Utilization Plan (WUP) has been developed based on site-specific information, careful evaluation of future samples is necessary. Should your records show that the current WUP is inaccurate you will need to'have a new WUP developed. The issuance of this COC does not excuse the Permittee from the"obligation to comply with all applicable laws, rules, standards, and ordinances (local, state, and federal), nor does issuance of a COC to operate under this permit convey any property rights in either real or personal property. Per 15A NCAC 02T .1304 and NRCS standards a 100-foot separation shall be maintained between water supply wells and any lagoon, storage pond,or any land application of waste. Please be advised that any violation of the terms and conditions specified in this COC,the General Permit or the CAWMP may result in the revocation of this COC, or penalties in accordance with NCGS 143- 215.6A through 143-215.6C including civil penalties,criminal penalties, and injunctive relief. If any. parts,.requirements,-or limitations contained in this COC are unacceptable, you have the right to apply for an individual permit by contacting the Animal Feeding Operations Program for information on this process. Unless such a request is made within 30 days,this COC shall be final and binding. In accordance with Condition 11.23 of the General Permit,waste application shall cease within twelve(12) hours of the time that the National Weather Service issues a Hurricane Warning,Tropical Storm Warning, or a Flood Watch/Flash Flood Watch associated with a tropical system for the county in which the facility is located. Sampling Criteria: a. Monitor well(s) MW-1, and MW-2, shall be sampled every March, July and November for the parameters listed below. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measuring points (top of well casing) of all monitoring wells shall be surveyed relative to a common datum. The water level shall be measured and recorded prior to purging the wells. The pH shall be measured and recorded prior to sampling for the remaining parameters: Nitrate(NQ3-N) Total Ammonia Nitrogen(NH3-N) Chloride Total Dissolved Solids (TDS) Fecal Coliform pH(field) 'Potassium Water Level Sodium b. Monitor well(s) MW-1, and• MW-2, shall be sampled initially to establish background concentrations, and upon permit renewal, for the paranieters in la. above and the additional parameters that follow. .Any_of the following parameters that exceed 15A NCAC 2L groundwater quality standard limits upon initial or renewal sampling must-be added-to--the-list-in,.l a. for routine monitoring: Copper Zinc Total Phosphorous c. If concentrations for any single parameter, or any combination of parameters,listed in or added to la. exceed 15A NCAC 2L groundwater quality standards in any single monitor well, or any combination of monitor wells, for three consecutive monitoring periods,then monitor wells MW- 1, and MW-2; shall be sampled thereafter every March, July and November for the group of - parameters listed in la. and lb. d. Any laboratory selected to analyze parameters must be Division of Water Resources (DWR) certified for those parameters required. 2. Reporting/Documentation: a. All reports and documentation(GW-1,GW-30, GW-59) shall be mailed to the following address: Division of Water Resources • Information Processing Unit 1617 Mail Service Center Raleigh,NC 27699-1617 Updated blank forms (GW-1, GW-30, GW-59) may be downloaded from the Water Quality Regional Operations Section's website_at http://portal.ncdenr.org/web/wq/aps/gwpro/reporting-._ - forms or requested from the address mentioned above. b. The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality Monitoring: Compliance Report Form) by the Groundwater Section (address listed in the "Reporting / Documentation" section of the Groundwater Requirements), on or before the last working day of the month following the sampling month. The data of all groundwater sampling analyses required by the permit conditions must be reported using the most recent GW-59 form along with attached copies of the laboratory analyses. , Additional Requirements: Any additional groundwater quality monitoring, as deemed necessary.
