Inspection Summary
On September 19, 2017, Edward Watson and Ori Tuvia of the MRO performed a compliance inspection for permit
WQ0006906. During the site visit, Brian Eades pointed out the new digester that is being built for the Taylorsville WWTP.
The new digester will have a 500,000 gallon capacity and increase the residuals storage for the WWTP to 10 months.
The digester design will include two blowers and difussion will occur through horizontal pipes with small diffusion holes
throughout the pipes. This new digestor is expected to be in service by the beggining of the new year.
The addition of this digester will also include a new residuals loading area. The new area incorporates a drain located at the
loading station designed to capture any residual over flow and return the spilled residuals to the head of the plant. However,
spill absorbant practices will need to be maintained during tanker loading events.

Dear Mr. Robinette:
Edward Watson of the Mooresville Regional Office (MRO) viewed an active land application event on May 1 Oth,
2017, at fields ID # 08-01. and 08-04 in Alexander County. The inspection was a review of the application process.
During the site visit, a review of the residuals loading area at the WWTP was performed. The presence of a stream
located near the facility presents concern for the MRO. Should a significant spill occur during the loading of
residuals, the residual material may potentially reach the stream and create an impact to surface waters. Currently,
it appears that there is no means of capturing residuals materials should a significant spill occur during the loading
of the residuals.
Several options may be employed as measures to prevent surface water impacts and capture spilled residuals, such
hay bales, an earthen berm; or a drain box.
Whichever means of capture is employed, Title 15A NCACO2T Operations and Maintenance .1110(4)(a) indicates
that spill control provisions need to be incorporated into the process.
Finally, a request was made during the site visit to have the Plant Available Nitrogen (PAN) values submitted as an
add information for the site visit.
This permit recently renewed in July 2016. The permit expires June 30, 2021.

impact to surface water. Spill rentention needs to be implemented at the WWTP.

The Mooresville Regional Office (MRO) conducted a compliance inspection on June 23`d, 2014,
which included a records review and facility visit. The MRO would like to thank Brian Eades for his
time during the inspection.
Please note your current permit expires on June 30, 2016. Per permit conditions a renewal
application is due 180 days prior on December 31, 2015. The MRO provided a complete copy
of the current permit to keep on -site at the WWTP; otherwise, records appeared to be in order and
the residuals storage area well maintained.

permit was located at the WWTP. Per Mr. Eades the complete permit was likely on file at the Town Hall.

2003 by Ms. Ellen Huffman with this Office.
The report indicates compliance; however, calibration records for the pH equipment used during
the lime stabilization process were not available at the time of inspection. The contractor needs to
supply the 'Fvwn with calibration records for the pH equipment used during the lime stabilization
process and include the information with the Federal Annual Report due to the EPA on February 17,
2003 and the State Annual Report due March 1, 2003.

Enclosed is the land application inspection report for the inspection conducted on
September 8, 2000 by Ms. Ellen Huffman with this Office. This inspection is part of our
initiative to inspect all land application programs during each calendar year.
The report indicates analytical compliance with your permit requirements; however,
Taylorsville has no dedicated residuals storage capability at the wastewater treatment plant. The
0.200 rules (Page 7 of 15 NCAC 2H .0200, section .0205 (c)(9)(h)(ii)) states "a minimum of 30
`~ days storage will be required on all facilities" and the storage units "shall be units separate from
\ the treatment system, i.e., not the clarifiers, aeration basins". With additional residuals storage,
the wastewater treatment plant operator will have more control over the solids within the plant
and it is less likely that the plant will loose solids via the effluent during wet weather.

The Town of Taylorsville has one wastewater treatment plant. Residuals are treated by aerobic
digestion. The Town usually removes sludge twice a year (in the spring and fall) from the aerobic
digesters. Other than the aerobic digester basins, there is no sludge storage. The 0.200 rules (Page
7 of 15 NCAC 2H .0200, section .0205 (c)(9)(h)(ii)) states "a minimum of.30 days storage will be
required on all facilities" and "shall be units separate from the treatment system, i.e., not the
clarifiers, aeration basins". Taylorsville runs a 30 day aerobic bench scale test (option 3) on the
digesters to prove Vector Attraction Reduction; and fecal coliform analyses for pathogen
reduction requirements. All lab data reflect compliance with the current land application permit.
The Town of Taylorsville was not actively applying sludge at the time of inspection. The last
field on which sludge was applied (in March 2000) was inspected and found to be in compliance
with the permit. There were no nuisance conditions noted and the residuals appeared to be evenly
applied with no signs of run-off.

Annual Report of land application activities. Additional information is required before we can continue our review.
On the Annual Pathogen and Vector Reduction Form you have checked Aerobic Digestion as the "Process to
Significantly Reduce Pathogens" and Option 4 as the Vector Attraction Reduction method. There is however no
supporting SOUR test data included in the 1997 annual report.
Permit condition.Ill. 5. requires that records ofstabilization processes utilized should clearly demonstrate
compliance with 40 CFR 503.32(b) and 40 CFR 503.33. Permit condition III. 6. requires that these records be
submitted as part of the annual report.
Please reference the state issued Permit Number when providing the requested information. All information should
be signed and submitted in triplicate to the attention of Kevin Barnett at the letterhead address. The information

our records show that Permit No. WQ0006906 was issued on
July 2, 1997 for the land application of sludge generated by
your wastewater treatment facility. The purpose of this letter
is to advise you of the importance of the Permit and the
liabilities in the event of failure to comply with the terms and
conditions of the Permit. If you have not already done so, it
is requested that you and other appropriate employees thoroughly
read the Permit.
The Permit sets forth specific performance standards,
operation and maintenance requirements, monitoring requirements,
including annual reporting and groundwater monitoring activities
to be performed by the Permittee, plus general conditions
applicable to non -discharge permits. Failure to comply with the
terms and conditions of the Permit subjects the Permittee to
enforcement action pursuant to Section 143-215.6 of the North
Carolina General Statutes. A civil penalty of up to $10,000 per
violation may be assessed for such violations.
Please note that the subject Permit expires on June 30,
2002. Part VI, No. 8 of the Permit requires that a renewal
request be submitted at least six (6) months prior to
expiration. Also the Permit is non -transferable until such time
that the Permittee has requested a name change to the new
Permittee.

Due to limitations in storage space, please submit the reports in bound form rather than
in notebook form. Permittees are subject to civil penalty for failing to submit the Annual
Report as required by their permit.
Enclosed are summary and certification sheets that must be attached to your annual
report. Please complete these sheets and if necessary make copies of the blank forms if
extra forms are needed for additional sites.
At the current time, the monitoring information that must be included in this report are
the requirements contained in your State issued land application permit.
You will however note that the report forms which are enclosed have been modified
from last year to include additional information required by EPA as outlined in 40 CFR Part
503. The information marked on the form related only to Part 503 is optional for the State
at this time unless it is specifically addressed in the State issued permit. However since the
requirements of 40 CFR Part 503 are self implementing, they are a legally binding and
enforceable federal requirement for all persons subject to the requirements of 40 CFR Part
503, even if these requirements are not currently contained in a State issued permit.

The State of North Carolina is currently negotiating with EPA to receive the federal
permitting authority and delegation to issue land application permits for EPA in our State.
This will allow you to avoid having two permits (one from the State and one from EPA) for
the same activity. While this delegation process is being completed, permittees legally are
subject to the new Federal requirements while, continuing to comply with their existing
State issued permit. At this time, the delegation process is progressing slowly and the
formal approval date for the delegation is unknown.

and three (3) copies of laboratory results are attached.
4. Annual TCLP analysis was performed and three (3) copies of certified laboratory results
are attached.
5. All other monitoring was performed in accordance with the permit and reported during the year as
required and three (3) copies of certified laboratory results are attached.
6. All operations and maintenance requirements were complied with or, in the case of a deviation,
prior authorization was received from the Division of Environmental Management.
7. No contravention of Ground Water Quality Standards occurred at a monitoring well or
explanations of violations are attached to include appropriate actions and remediations.
8. Vegetative cover as specified in the permit was maintained on this site and the crops grown were
removed in accordance with the crop management plan.
9. No runoff of residuals from the application sites onto adjacent property or nearby surface waters
has occurred.
10. All buffer requirements as specified on the permit were maintained during each application
of residuals.

Dear Mr. Duncan:
Our records show that Permit No. WQ0006906 was issued on
November 13, 1992 for the land application of sludge generated
by your wastewater treatment facility. The purpose of this
letter is to advise you of the importance of the Permit and the
liabilities in the event of failure to comply with the terms and
conditions of the Permit. If you have not already done so, it
is requested that you and other appropriate employees thoroughly
read the Permit.
The Permit sets forth specific performance standards,
operation and maintenance requirements, monitoring requirements,
including annual reporting and groundwater monitoring activities
to be performed by the Permittee, plus general conditions
applicable to non -discharge permits. Failure to comply with the
terms and conditions of the Permit subjects the Permittee to
enforcement action pursuant to Section 143-215.6 of the North
Carolina General Statutes. A civil penalty of up to $10,000 per
violation may be assessed for such violations.
Please note that the subject Permit expires on October 31,
1997. Part VI. 10. of the Permit requires that a renewal
request be submitted at least six (6) months prior to .
expiration. Also the Permit is non -transferable until such time
that the Permittee has requested a name change to the new
Permittee.
