On August 23, 2010 the Division of Water Quality(DWQ)received your application sets in response to a
letter from DWQ dated July 27, 2010 requesting more information for the above referenced project. The
DWQ has determined that your application was incomplete and/or provided inaccurate information as
discussed below.
Additional Information Requested:
® This project will require a permitting fee of$570 based on the reading of Water Quality
Certification 3821 (NW29): Id. "Total stream impacts equal to or greater than 150 linear feet
of intermittent and/or perennial stream to be filled, culverted, rip rapped or relocated,
including temporary and/or permanent impacts." A check will be needed payable to Division
of Water Quality to continue the review of this project.
Please submit this information within 30 calendar days of the date of this letter. If you will not be able to
provide the requested information within that timeframe, please provide written confirmation that you
intend to provide the requested information,and include a specific timetable delineating when the
requested materials will be provided. If we do not hear from you in 30 calendar days, we will assume that
you no longer want to pursue this project and we will consider the project as returned. Please be aware
that any impacts requested within your application are not authorized (at this time)by the DWQ. Please
