Johnson Brothers -Mutual Distributing was not approved for the following reason:
The NOT Certification form was not uploaded (do not upload a PDF of the on-line form. There is a link to the
form you need to complete and sign underneath the upload button). Also, you indicated that there was no
approval of the E&SC plan, but there is an approval letter from the City of Charlotte dated 4/1/2019 in the
permit records (see our on-line repository). You must provide close-out documentation from the City. Please
resubmit a new NOT request with these items addressed.
