As part of our EMS, we are required to inform you of the EMS's existence and also provide you with a copy of our environmental policy. You may have received a copy of the policy upon beginning work with Gastonia. If so, please disregard the previous policy that you received and replace it with the attached revised version. The attached policy revision was approved by City Council on November 19. We would request that a copy of this policy be forwarded to all of your employees who might visit or work at any of our facilities. Anyone performing work or visiting any of our facilities should check in at the facility's main office upon arrival. We would also ask that, prior to conducting any work or service, your employees check with plant personnel concerning any EMS procedures or requirements that their work may involve. Please address the following by December 29, 1997: 1- Your project narrative states that the emergency equalization (EQ) basin (current polishing pond) will only be necessary less than once per year based on maximum projected flow. Is this maximum projected flow based on existing flows in the Catawba basin or does this consider a "built-out" Catawba basin? If this is based upon existing flows, please evaluate the system and determine how many times per year the emergency EQ basin would be utilized with the flows generated in a "built-out" Catawba basin. How many times per year do you project that the EQ basin (current aeration basins) will receive wastewater flows based upon "built-out" flows? 2- The Division has concerns over the removal of the existing aeration equipment in the proposed EQ basin. The use of chemical odor control is good for odor control, but how will such a large slug of potentially septic wastewater (from the EQ basin and emergency EQ basin) affect the treatment at the Long Creek wastewater treatment plant (WWTP)? 3- Based upon my calculations,estimating the rainfall in the area as 40 inches per year and estimating the area of the EQ basin and emergency EQ basin to be approximately 3.75 acres, there is over 4 million gallons per year of rainwater that will have to be transported to the Long Creek WWTP for treatment and disposal. If the emergency EQ basin is only estimated to be used less than once per year, the majority of this flow would be tributary to the WWTP within a short period of time. Please evaluate this and verify that the EQ basins would be able to equalize this flow over a long enough period of time that no flow violations would occur at the Long Creek WWTP. 4- The Division has concerns over the upward slope of the drain line from the emergency EQ basin back into the wet well. Please explain how you intend to eliminate solids buildup in this drain line from the emergency EQ basin back into the wet well. I have seen the 6 inch flush line coming into the 12 inch drain line close to the wet well (near the steepest upward slope), but have concerns of it being able to maintain a 2 foot per second velocity in a 12 inch line. It appears as though the wastewater would flow back into the wet well through the 24 inch overflow line just as easily as it would flow through the 12 inch line. Without a profile view of the 24 inch line,it is hard to tell the configuration of it between the vault and the wet well. There is a 90 degree elbow shown on the overflow line which appears to go upwards which would force the wastewater through the 12 inch line,but the invert of the 24 inch line in the wet well is shown as 631.12 which insinuates that the elbow is pointed downwards. Please discuss the flow of the wastewater from the EQ basin back into the wet well. 6- Your project narrative states that the invert of the 24 inch drain line in the wet well will be set at the high water alarm, however the plans show an invert of 631.12,the bottom elevation of the wet well. Please eliminate this discrepancy. 7- The 24 inch overflow line is shown with a manually operated knife valve. What will be the normal operating position of this valve? The Division has some concerns over both of the following scenarios. If this valve is left open,based upon the invert level shown in the wet well, the EQ basin will really act as an extended wet well. If normally in the closed position, the valve will have to be manually opened upon sounding of the high water alarm. 8- Is the proposed emergency EQ basin lined? 9- The Division requires that all pump stations and force mains be permitted, as a pressure system, and covered under one permit. Therefore the force main must be permitted with this permit action. To permit the force main, a complete set of construction drawings,including plan and profile views of the force main, is required. Work on the bottom of the slab for the clarifier is in progress. • Final Clarifier No. 1 has been hydrostatically tested. The clarifier passed the test and backfilling around the structure is in progress. • The under-slab piping for the filters is complete. • The elevator base,mud slab,and under-slab drain piping from the elevator to the sump pit at the Main Lift Station are all complete. .• The under-slab piping for Clarifier No.3 is complete. • Flushing water lines have been installed from the southwest corner of the drying beds to within approximately 100 feet of the manhole. • Manholes Nos. 5, 6, 17,and 18 have been installed. • The 24-inch ductile iron pipe from Manhole No.2 to Manhole No. 10 has been installed. • The eight-inch ductile iron pipe from the solids contact reactor to Drain Manhole No. 10 has been installed. • The three-inch domestic water line to the new potable water well has been installed. • The 48-inch ductile iron pipe under Manhole No. 1 has been installed. • The eight-inch ductile iron pipe raw sludge line from Manhole No. 1 to the existing raw sludge pump station has been installed. • Contract No.2 (Electrical)Watson Electrical Construction Company • Work on the installation of the exposed conduit in the Lab/Control Building Mechanical Room is in progress. Preston Howard,Jr.,PE Page 9 July 7, 1997 • Work on the construction and erection of the service pole and the service drop conduit is in progress. • The underground conduit at Substation No.4 has been installed. • The rough-ins in the Blower Building are under way. • The installation of the lighting at the Blower Building has been completed. • The ductbank at the existing Blower Building has been completed. • Contract No.5 (54-Inch Outfall) BRS,Inc. • All work is complete except for some remaining restoration work. • The concrete headwall has been constructed and riprap has been placed on the creek bank from the headwall to the creek. • A temporary bulk head has been installed in the line to prevent the river from surcharging the outfall line until such time as it is placed in service. The line was tested and it passed the test requirements. 02/06/96 • A conference was conducted at the office of the A/E in Charlotte to review the schedule prepared by the General Contractor and to give other Prime Contractors an opportunity to identify any conflicts, problems, or other issues. The following is a summary of the key items that were discussed. • The contract period is 30 months for all Prime Contractors. The General Contractor has prepared a schedule requiring completion of the work by all Prime Contractors in 24 months. At the time of the meeting, the General Contractor was not willing to change the 24-month schedule. • The General Contractor is presently six-to-eight weeks behind schedule, but he anticipates being able to make up the lost time. The General Contractor attributes a large portion of the delays,to date,to bad weather. • The Electrical Contractor's representative stated that they could not complete the work in 24 months and they have serious reservations about the General Contractor's ability to complete the work in 24 months. The Electrical Contractor's representatives further stated that they had serious reservations, based on the progress to date, about their ability-as well as the ability of the General Contractor - to complete the work within the 30-month contract period. • The Electrical Contractor presented a detailed review of the General Contractor's schedule and the associated electrical manpower requirements. The bulk of the electrical work, as presented by the Electrical Contractor, has been compressed into a four-to-five month period necessitating as many as 50 workers per month. The Electrical Contractor said that he could not meet this requirement. • The A/E advised the Contractors that the NC State Statutes for multiple- Mr.A. Preston Howard,Jr.,PE Page 10 July 7, 1997 prime contract construction are applicable. The A/E further advised that the General Contractor, as project expediter, has the responsibility to prepare a construction schedule for the entire project for all Contractors. The General Contractor also has the responsibility to cooperate with and obtain from the other Prime Contractors their respective schedules and to integrate them into a Project Progress Schedule which will show,by detailed CPM, the projected progress from start to finish within the allotted time frame. • The A/E STRONGLY recommended that all of the Prime Contractors sit down together and work until they can develop a schedule on which each Prime Contractor is willing to "sign-off" and that the schedule reflect the actual projected progress and completion date. The A/E expressed the opinion that if the schedule issues are not resolved immediately, the project is headed for serious problems and the project progress will be seriously impacted because the Prime Contractors will be "fighting" each other rather than cooperating to get the work completed. • The General Contractor's representative expressed his opinion that the "smaller" Prime Contractors would never agree to, or "sign-off" on, any schedule which shows an early completion because the "smaller" Primes would then incur liability. • The General Contractor's representative advised that the current schedule prepared by the General Contractor, which requires early completion by all Prime Contractors, cannot be changed without approval from Upper Management at the Home Office. • The A/E asked the General Contractor to request the necessary approval to revise the schedule and to report on the Home Office's response at the scheduled February 14, 1996, Construction Progress Conference. Regardless of the Home Office's decision, the Contractor was asked to prepare a schedule which reflected the actual projected progress of the other Primes. • The Plumbing and Mechanical Contractor's representative expressed his concern over the ability of all Prime Contractors to meet the early completion date as well as his own ability to meet the early completion date as a Subcontractor to the General Contractor for the mechanical work. • In conclusion, the A/E requested that all the Prime Contractors begin working together now to resolve the scheduling problems; and that the General Contractor,as project expediter,take the lead in this effort. 03/13/96 • Contract No. 1- (General) Balfour Beatty,Inc. • The exterior walls of the aeration basins were completed and some work was accomplished on the interior reinforced concrete walls. • Drying-in of the Blower Building was completed. • The erection and welding of the structural steel roof framing for the Mr. A. The roof framing is currently ready for placement of the standing-seam metal roofing, and the standing-seam metal roofing is stockpiled on site. • The reinforced concrete walls on Clarifier No.3 were completed. • Hydrostatic tests were conducted on Clarifiers No. 2 and No. 3. Each clarifier passed the test. • All under-slab piping at the filters was installed. The reinforcing and forming for the filter bottom slab was completed and the placement of the concrete was taking place (approximately 1,100 cubic yards). • One of the four pours for the walls at the Main Lift Station up to Elevation 647 was completed. • The roof drain system for the Blower Building was installed. • The installation of the new water line for the new well was completed. The General Contractor was waiting for the Electrical Contractor to provide the power and control wiring. • Several catch basins and sections of drain pipe were installed. • Contract No.2-(Electrical) Watson Electrical Construction Company • The switchgear at the Blower Building was set. • Three sections of MCC4 were installed. • The blower alarm panels were installed. • The exterior lights at the Blower Building were installed. • The exterior lights were installed on the Lab Office Building, along with five combination starters,panel interiors,Panel LOLA,and Panel LOLB. • Some of the work for the controls for the well pump was completed. • Some of the excavating, undercutting, and backfilling for the Generator Facility was completed during the previous month. Lighting and disconnects for associated equipment are on hold until Atlantic Coast Mechanical completes their work. • The generator was checked out and prepared for start-up. • The Main Lift Station feeder conduits and cable trays were set in MC Sections. Disconnects were installed at the Preliminary Treatment. • DAF lighting was installed on the Main Floor. 03/97 • Contract No. 2- (Electrical)Watson Electrical Construction • In the Main Lift Station, the Contractor completed setting the switchgear and wiring and completed the lighting. He is currently working on control conduits and elevator power. In addition,the pumps were wired. • The installation of the conduits for the final clarifiers were completed along with the installation of the disconnect switches and emergency stop switches. • In the Chlorine Storage and Feed Building,the Contractor wired the hoist and completed all the general building wiring. At the time of the meeting, the Electrical Contractor was waiting for the General Contractor to complete equipment installation in the facility so that electrical work could be completed. • At the aeration tanks, the Contractor completed all the conduit tie-in's to the air metering vaults and the wiring for the recirculation pumps back to the Blower Building. • In the Chemical Feed Building,the Contractor worked on conduit and wiring to the pumps. At the time of the meeting, he was waiting for the General Contractor to complete equipment installation. Mr. A. The contract has been completed. Final payment has been made and the contract is closed out. • Contract No.4-(Mechanical)Atlantic Coast Mechanical,Inc. The Contractor reported that no work had been performed during the previous month. • Contract No.5-BRS,Inc. The contract is complete and has been closed out. CROWDERS CREEK WWTP EMC WO NO.93-016 12/31/94 • The Industrial Waste Survey was completed by the City of Gastonia Staff. 02/17/95 • Results of tests performed by Sierra Environmental were received by the City Mr.A. This study evaluated polymer feed to remove heavy metals at the plant. 03/29/95 • In-line testing of acute toxicity within the collection system was performed. Results are currently being evaluated and more testing is proposed to establish a baseline for waste characteristics. 04/06/95 • A meeting was held with the City Staff, AWARE, Inc., and J.N. Pease Associates to determine a full-scale implementation of polymer feed at the Crowders Creek WWTP. A letter was submitted to the regional office requesting approval with a start date of May 1,1995. 05/01/95 • The City of Gastonia began implementing a polymer feed system at the Crowders Creek Wastewater Treatment Plant on a full-scale basis. Initially, 2.0 ppm was fed at the plant and analyses were performed on five major metals to determine the percentage of removal. A microscopic review of the wastewater was performed to determine the effects of the polymer on phosphorus removal. The polymer was increased during the testing period from May 1, 1995 through June 15, 1995 from a 2.0-ppm to a 20-ppm feed rate. The test results are enclosed with this report. Preliminary findings indicate that metals are not the primary cause of toxicity at the Crowders Creek Wastewater Treatment Plant. The City decided to continue in-line testing of wastewater at key locations within the Basin in an attempt to determine the cause of toxicity. 05/09/95 • Performed SIU toxicity screenings for the following industries: (1) Amp; (2) Kings Mountain; (3)Rauch; (4) Sunshine;and(5)Threads. 05/31/95 • The City of Gastonia received approval of the Industrial Waste Survey. 06/18/95 • Began week-long SIU toxicity screenings for Threads and Ritchies. 09/08/95 • Met with Walt 011is, Director of Public Utilities of Kings Mountain, and discussed chronic toxicity screening for the dischargers in the Kings Mountain system. This work will be, scheduled and performed by Kings Mountain in the near future. 09/17/95 • A meeting was held with the City staff,Aware,Inc., and J.N. Pease Associates to review results of the polymer testing at Crowders Creek WWTP. The results were not conclusive that polymers would resolve toxicity at Crowders Creek WWTP, and the City began negotiating for a pilot plant evaluation. Negotiations were completed and implementation of a pilot plant will begin during October 1995. 10/03/95 • Performed toxicity screening and results are pending. 10/17/95 • Core team met to discuss the toxicity screening results. Results were not conclusive;additional test scheduled. 11/07/95 • Core team met to discuss the toxicity screening results. Efforts will concentrate on the treatability of the wastewater at Crowder Creek WWTP. 12/14/95 • Core team met to discuss toxicity. It was decided to include Crowder Creek flow in the pilot plant study. Aware is suppose to submit a proposal to Gastonia for the additional work. This work will begin when the pilot test is complete at Long Creek facility. The purpose is to determine treatability of the wastewater at Crowder Creek WWTP. The toxicity test passed at this facility in December. In plant testing had to be discontinued until toxicity failed. The reason is that a failed sample is used as control for testing. January, • Currently, the Crowders Creek WWTP is meeting its toxicity limits and is not February, in violation at this time; therefore, additional testing cannot be conducted at and this facility until a violation, on which an analysis can be run, occurs. The March, Core Team continues to monitor the situation and will act accordingly. 1996 April- • The Crowders Creek WWTP is meeting its toxicity limits and is not in July 1996 violation at this time. A request to eliminate the SOC has been sent to the State,and the City is currently waiting for a response. CATAWBA CREEK WWTP EMC WO NO. 93-017 06/15/94 • The plans and specifications were completed and submitted to DEM for review, approval, and funding. The improvements consisted of upgrading the existing facility with a lime addition facility and piping modifications. 09/16/94 • Plans and specifications were advertised for bids. 10/12/94 • Bids were received and tentatively approved by the City Council. 11/17/94 • DEM approved plans and specifications for construction. 12/06/94 • Bids were approved by DEM. 12/31/94 • The Industrial Waste Survey was completed by the City of Gastonia Staff. 02/22/95 • The SBF approved the funding of the project. 02/23/95 • Contracts were executed for construction of the project. 03/27/95 • An updated Headworks Analysis was submitted to the NCDEHNR/DEM Pretreatment Group. 04/11/95 • The Industrial Waste Survey Summary was submitted to the NCDEHNR/DEM Pretreatment Group. 04/19/95 • The Monthly Progress Meeting was held at the construction site. Hickory Construction Company and John Bisanar, Inc.,were present for this meeting. Progress made during the previous month included excavation for the foundation, tests on the bearing pressure of the soil, and doweling the Mr.A. The Contractor also submitted shop drawings for the project. 04/20/95 • The City of Gastonia received approval of the Headworks Analysis from the NCDEHNR/DEM Pretreatment Group. 05/17/95 • The Monthly Progress Meeting was held at the construction site. The Contractor had finished the excavation of the foundation for the Bulk Lime Station and the Acid Tank slab. The Acid Tank slab was poured and work began on the Bulk Lime concrete slab. A list of shop drawings was presented to the Contractor for submittal. 05/31/95 • The City of Gastonia received approval of the Industrial Waste Survey. 07/19/95 • The Contractor is installing the 30-inch DIP air line. • Performed SIU toxicity screenings for the following industries: (1)Arnold; (2) GMH; (3) Scott;and(4)Spring Ford. 07/25/95 • Performed SIU toxicity screenings for the following industries: (1) Choice; (2) Firestone; (3) IEP; (4)Modena;and (5)Uniroyal. 09/20/95 • A second crew has started to work on the Digester and Chemical Feed System. • The air piping is complete and has been tested. • Work will continue on the tie-in of the 30-inch DIP. • Envirex equipment to ship August 25, 1995. • Zimpro equipment to ship September 25, 1995 • Work on the yard piping will continue during the next month. • Manhole No.50 is due to arrive next week. • Manhole No.51 will be shipped three weeks after approval. • Pump, pipe insulation, and thermal protection shop drawings are currently under review. • The acid tank is to be delivered at the end of the month. • Tapered insulation for the roof curb around the perimeter will work satisfactorily. • The Roofing Subcontractor has already reviewed the existing roof conditions. • Clarification Note No. 2, for changing circuit breakers for the blower, has been issued. 09/20/95 • The blower installation is scheduled for between November 1 and November 15, 1995. The schedule indicated test location, parameter, and frequency of test. 10/31/95 • Began instructional session with city staff on procedures and process control for the start-up period. Actual classes were held on site. 11/02/95 • Discussion took place regarding the use of Microtox Testing for acute toxicity. It was decided not to use Microtox Testing at this time. 11/07/95 • Core team meeting. Discussed new jar test results which will determine the start-up feed rates. 11/27/95 • Start-up lime feed system at Catawba WWTP. Began in plant testing program to determine toxicity reduction. 01/96 • We met with Mr. Steve Tedder and Ms. Colleen Sullins to request an extension of our compliance date within the SOC. As a result of that meeting, a request for an extension of the compliance date was made to the State. During this period,the City can optimize the chemical feed into the system to operate the facility at its peak level. 04/11/96 • As of February 28, 1996,the facility has been in substantial completion. 05/28/96 • The Core Team met with the Mooresville Regional Office and discussed reasons for toxicity failures. There main events occurred as potential causes: (1) equipment failure at the chlorination point caused excessive levels of chlorine in the effluent; (2) high influent concentrations of zinc potentially caused toxicity; and (3) over-feeding of lime to accelerate compliance caused high pH and toxic conditions. The Regional Office suggested that the City request a 30-day extension of the current SOC. 06/12/96 • A conference call among the Gastonia,staff, Mooresville Regional Office of the DEM, and the State Office of the DEM occurred. The discussion concluded with extending the SOC to August 15, 1996. 06/30/96 • The Catawba Creek WWTP passed two consecutive toxicity tests.
